Position Summary


Reporting to the Director of Assisted Living, the Assisted Living Supervisor is responsible for the front-line supervision of the Assisted Living Care Services staff.

This position will provide leadership, guidance, and quality care within the Assisted Living Care Services program.


Some Key Responsibilities

  • Organize and supervise personal support care services provided to residents by all personal support workers;
  • Monitor and ensure that clients are receiving care according to their service plan. This includes, but is not limited to, adjusting service plans as needed to meet the needs of our clientele;
  • Responsible for all day to day care services;
  • Monitor the Health Partners Gateway (HPG) system for client service frequency changes additions and omissions of LHIN services;
  • Document cancelled or missed visits to the North East LHIN Care Coordinator when they occur for reporting and quality monitoring purposes;
  • Assist the scheduling department with troubleshooting proper staff coverage;
  • Orientate new employees to the department and monitor their progression throughout their probationary period;
  • Complete and meet with staff for Performance Appraisals on an annual basis;
  • Coach staff on matters related to resident care, performance, and relations. Document the coaching, and notify the Director of Assisted Living of the event and resolution;
  • Assist clients with calls to the pharmacy; to renew their prescriptions, get new prescriptions, or change their prescriptions;
  • Interact with resident and/or families to resolve concerns; refer to the Director of Assisted Living if unable to resolve;
  • Develop modified routines for Return to Work employees in conjunction with the Director of Assisted Living and the Human Resources Director;
  • Set up and coordinate the Supportive Housing Falls Program;
  • Provide leadership to the team ensuring that the Assisted Living program operates efficiently and effectively;
  • Adhere to all established department policies and procedures regarding quality assurance, risk management, safety, infection control and emergency measures;
  • Communicate and interpret current and revised standards of care to staff and community partners;
  • Consult with physicians, Director of Assisted Living, members of other disciplines, community partners, residents, and families regarding resident care and programs; report any concerns or suggestions
  • Participate in the development and implementation of a CQI (continuous quality improvement) and Risk Management program within the Assisted Living department;
  • Maintain a thorough working knowledge of the Finlandia Health and Safety Manual, policies and associated Provincial Legislation;
  • In conjunction with the Director of Assisted Living, the Supervisor ensures that governing bodies and contractual obligations; including, but not limited to, quality initiatives and measures are met;
  • Participate on various internal and external committees as required;
  • Provide "on call" support to PSW's related to resident care as required;
  • Perform other related duties as required

Education

  • Postsecondary diploma or degree in Practical Nursing;
  • Registered and in good standing with the College of Nurses of Ontario

Experience

  • Experience in a Supportive Housing/Community care setting is considered an asset

Qualifications

  • Valid CPR Certificate;
  • GPA Certificate;
  • Finnish speaking is considered an asset
  • An acceptable Police Vulnerable Sector Check verified within the past six months
  • 2step TB test within the past six months
