Job Summary:

The Professional Practice Coordinator works in collaboration with Hospice's Clinical Leadership to support the professional development of all RN, RPN and PSW staff at the Hospice Residences and Palliative Medicine Program.

The Coordinator must be a strong leader with clinical expertise who plans and executes the Hospice vision within the organization and the community, displaying dedication to Hospice values.

The Coordinator will be responsible for leading clinical practice and quality improvement projects, providing training, education and mentorship, and developing policies, procedures, frameworks and tools to ensure all clinical staff have the skills and competencies to provide exceptional care.

This role works directly with the Director of Clinical Services and Informatics in collaboration with the Hospice Residence and Palliative Medicine Program Managers, and provides support when required.


Principal Duties and Responsibilities:


  • Support the Clinical Leadership team in developing and establishing annual and longterm goals and objectives to fulfill the mandates of the Hospice Residences and Palliative Medicine Program (PMP).
  • Lead program development initiatives to meet departmental goals and targets.
  • Engage in interdisciplinary collaboration and research with internal and external partners to identify bestpractices in providing quality, clientcentred and evidencebased care.
  • Develop, train and implement practices and projects which ensure best practices in the Hospice Residences and PMP. This includes providing staff education internally on a oneonone basis or in a group setting.
  • Monitor and evaluate the success of and adherence to new initiatives.
  • Create and maintain policies, procedures, toolkits and frameworks to support the implementation and sustainability of evidencebased practices and procedures.
  • Provide statistical analyses and prepare reports for the Clinical and Senior Leadership teams in relation to clinical programs, projects and initiatives to reflect performance against objectives.
  • Develop and maintain an effective orientation program for new Hospice Residence and PMP staff. Facilitate Day 2 of orientation for staff including Cerner electronic medical record training. Delegate mentor roles to existing staff based on strengths to ensure new hires are sufficiently prepared in their roles.
  • Collaborate with managers to arrange preceptors and orientation schedules.
  • Act as a consultation resource for the clinical staff providing care at the Hospice Residences. Provide ongoing guidance and education to staff, volunteers, new physicians, residents, fellows and students on an ongoing basis. Provides bedside nursing procedure training and support as needed at both Windsor and Erie Shores campuses.
  • Serve as a Cerner Super User for the organization, acting as a resource for all clinical staff.
  • Audit individual residence and PMP patient charts on a regular basis to ensure that information is current, accurate, and complete.
  • Facilitate the delivery and maintenance of ongoing and annual education and skills training and testing, including annual and new hire competency skills checklists for Hospice Residence and PMP staff in accordance with Hospice requirements and CARF accreditation standards.
  • Participate in CARF accreditation preparation.
  • Engage with external stakeholders through the organization of education events, inservices and other community events. Design and implement education strategies/ programs for community nursing agencies and other community partners.
  • Participate as a mentor for nursing students and medical students /residents/fellows in field placement education.
  • Assist with interviewing potential employees.
  • Contribute to probationary reviews and annual performance appraisals in collaboration with Hospice Residence Managers.
  • Share oncall manager responsibilities on weekends or after hours in rotation with the Hospice Residence Managers. This may require frontline nursing or PSW coverage in urgent situations where a shift cannot be replaced.
  • Provide backfill coverage of Hospice Residence and PMP Managers for vacations/LOAs.
  • Report to the PMP department to fulfil PMP nursing responsibilities when there are no schedule vacation or education/training days.
  • Provide backup support for nursing roles in PMP as scheduling allows, or for frontline nursing or PSW roles in the Hospice Residence in situations of urgent staffing shortages (sick calls or shift fills).
  • Attend any meetings, conference etc. for the purpose of coordinating services and/or professional development.
