The Role and Opportunity:


The Canadian Mental Health Association Toronto offers intensive case management services as part of the continuum of mental health services and supports for people with mental illness.

We are currently seeking a Concurrent Disorder (CD) Specialist to service as a clinical member of our Safe Bed, Home for Good, and Lurie Housing programs.

and is responsible for providing community support services to clients who have a concurrent disorder. The CD Specialist will have the clinical skill and experience to assess, plan, develop, co-ordinate, and provide treatment, rehabilitation and support services to the program clients with concurrent disorders (i.e. mental illness and addiction problem).


Salary Range:
$57,928 - $67,119


Work Hours:35 hours per week

Responsibilities

  • Assessment, service planning and discharge planning
  • Clientcentred engagement, service provision and intervention
  • Providing skill development, linkages, resources, information and advocacy
  • Crisis prevention and intervention
  • Develop crisis plans in collaboration with clients, prevention team and other relevant stakeholders
  • Assist with finding and maintaining affordable housing.
  • Maintaining trusting, respectful and collaborative working relationships
  • Develop linkages and coordinate services with client's inpatient and outpatient addiction treatment programs, residential facilities, selfhelp program (e.g. Alcoholics Anonymous, etc.)
  • Provide direct clinical services including individual and group supportive therapy.
  • Consultation to other staff on addictions related issues

Experience & Education Required

  • Minimum undergraduate degree/diploma in Social Work/Social Science or related field
  • Minimum 3 years of experience with direct service experience in community mental health and addictions
  • Experience and knowledge working within the Residential Tenancy Act
  • Experience in organizing and conducting behavior modification and support groups
  • Strong assessment, service planning and crisis intervention skills
  • Work experience and knowledge of concurrent disorders, homelessness, recovery principles and the criminal justice system as it relates to mental health
  • Strong organizational and time management skills
  • Ability to work collaboratively in a team environment
  • Excellent communication, listening, and interpersonal skills
A valid Ontario driver's license is required. Successful applicant must have access to a car and insurance for workplace use.
