Overview
Education
- Master of Social Work from an accredited School of Social Work, or equivalent degree as recognized by the Ontario College of Social Workers & Social Service Workers required.
Qualifications
- Recent experience in Geriatrics required
- Demonstrated ability to provide comprehensive assessment and treatment planning
- Demonstrated ability to advocate for services to meet complex needs of specific patients
- Demonstrated ability to provide educational and supportive counseling
- Demonstrated ability to provide resource identification, referral and linking
- Knowledge of health care legislation (Health Care Consent Act, Public Hospitals Act, Personal Health Information Act), policies, programs and issues related to social welfare institutions and community services.
- Excellent crisis intervention and mediation skills.
- Engages in ethical conduct and strives to understand and appreciate the diversity of our patient/staff population and community
- Computer skills required; ability to manage electronic recording, workload and meet submission timelines.'
- Participates in reflective practice as per College standards.
- Demonstrated ability to provide collaboration and consultation with interprofessional team and community based service providers
- Demonstrated ability to work as a member of an interdisciplinary team
- Participate in education to various groups including social work and other health professionals, students, patients and families, colleagues and community agencies
- Participate in professional, program and hospital development, planning, implementation and evaluation of service.
- Strong written and verbal interpersonal and communication skills required
- Demonstrated ability to prioritize and work effectively in a fast paced environment required
- Demonstrated patient centered philosophy of care provision
- Demonstrated ability to welcome change and manage it innovatively
- Good work and attendance recordrequired
