    CollingwoodGeneral and Marine Hospital (CGMH) is an 84 bed hospital located inCollingwood, Ontario (approximately 90 minutes north of Toronto) that servesmore than 74,000 permanent residents and 3.5 Million annual visitors to thecommunities of Wasaga Beach, Collingwood, Clearview and the Blue Mountains.CGMH is an acute care hospital providing emergency care, diagnostic servicesincluding lab, imaging and cardio respiratory therapy, as well as two inpatientunits (medicine and surgery). In addition, CGMH also provides care in specialtyareas including obstetrics, orthopaedics, intensive care and surgery.     The Opportunity:

    TheSocial Worker will provide clinical social work services throughout theorganization, including Psychosocial assessments, end of life support,assessing eligibility for services, the development and coordination ofcomprehensive discharge plans, and multidisciplinary team consultation. Theindividual will be responsible for providing direct clinical services topatients within the assessment and treatment framework of social work knowledgeand skills.

    Qualifications:

  • Must have a Master's Degree in SocialWork
  • Must be registered with the OntarioCollege of Social Workers and Social Service Workers
  • Must be a member of the OntarioAssociation of Social Workers
  • Must have a minimum of 3 years ofexperience
  • Must have knowledge of and appreciationof the diagnostic and treatment procedures utilized by related disciplines
  • Must strive to continuously provide asafe and supportive environment for patients, staff, physicians, and volunteers
  • Must adhere to the duties of workers, asstipulated in the Occupational Health and Safety Act
  • Must have familiarity with communityresources, LTC placement process, and relevant consent and capacitylegislation.
  • Experience and certification in ManagingAggressive Behaviour and/or Gentle Persuasive Approach is considered an asset.
  • Must have an ability to effectivelycommunicate (written and verbal) in the English language in order to provideexcellent communication with team members, patients and families. Ability tocommunicate in additional language(s) is considered an asset.
  • Must have basic computer skills
  • Must be dedicated to professional growth& development
  • Must be able to perform all bona fideand essential duties of the position, which may include physical demands suchas moving/transferring patients, pushing, pulling, bending, etc.
  • Must have demonstrated excellentattendance/performance record
  • Excellent organization andprioritization skills
  • Ability to work independently andeffectively communicate with other members of the health care team
  • Duties and Responsibilities:

  • Assessing patients for the purpose ofevaluating the need for service/resources and formulating recommendations ortreatment plans within various areas of the hospital including (but not limitedto) all inpatient areas and the Emergency Department.
  • Coordination of discharge planning withhealthcare team, caregivers, and community partners.
  • Provide consultation to physicians,nurses, and other relevant disciplines concerning the social and emotionalfactors which contribute to patient's treatment.
  • Maintain close communication with thephysician and other professionals directly involved with the patient's circleof care.
  • Assess the psychosocial needs ofpatients and to help in the development of care plans to meet these needs.
  • Act as an advocate on behalf of patientsand their families, both in hospital and in the community.
  • Act as a liaison between hospital andcommunity programs related to the needs of our patients.
  • Lead, support, or participate in keyinitiatives that align with the strategic direction.
  • Active member of applicable careteam/unit/department meetings and huddles.
  • Assess eligibility for resources andprograms with external partners and community agencies.
  • Implementation of intervention planswith patients, family members, and the multi-disciplinary teams.
  • Active participation in the Hospital'sHealth and Safety Internal Responsibility System, which includes maintainingaccountability for the defined responsibilities listed in applicablelegislation, policies and procedures.
  • Actively contributes within the scope ofthe position to the enhancement of the patient experience through continuousimprovement, learning, and teamwork.
