About Program:

Rapid Response Saving Lives (RRSL) is a pilot project. This program offers pre-emptive mental wellness support and post-hospital discharge support to individuals and their family members. The program is based on the principles of hope, recovery, empathy, self-determination, and respect.

Case managers and Peer Support Workers will work with the clients experiencing substance abuse and mental health issues and ready to work toward their recovery.

RRSL staff will provide linguistically and culturally competent mental wellness psycho-educational support and post-hospital discharge support to patients and their families to achieve their recovery and rehabilitation goals.


About Position:


This position will focus on building addiction and mental health focused peer support that is integrated with current programs and partnerships.

Program is offered to family members of individuals experiencing substance abuse and mental health issues and ready to work towards their recovery.

Peer Support Worker will provide linguistically and culturally competent support to family members. This program is based on the principles of hope, recovery, empathy, self-determination, and respect.

This position will work to enhance existing services offering a safe, non-judgmental environment through a guided journey for the individual and their family members.


Qualifications:


  • Degree or diploma in a health services or related field. Preferred education in Addictions,
  • Mental Health or Social Work.
  • Experience of group work is preferred
  • Solid working knowledge of recovery principles
  • Effective interpersonal, communication and organizational skills to function effectively
- within a collaborative team environment

  • Ability to work independently with mínimal supervision
  • Knowledge of community services and supports available in Region of Peel
  • Excellent working skills of MS Word, Excel, PowerPoint and internet
  • Knowledge of DATIS, Catalyst, Ontario Common Assessment of Need (OCAN) is an asset
  • Fluency in Punjabi, Hindi and Urdu languages is preferred

Requirements:


  • Must possess a valid Ontario Driver's License, have regular access to a reliable vehicle and provide proof of adequate vehicle insurance
  • A satisfactory current Vulnerable Sector Screening (Criminal Record Check) report
  • Current CPR and First-Aid certification
  • Vaccines (COVID19 and others) are encouraged.

Key Responsibilities:


  • Establish a supportive relationship with clients and their families
  • Explain the Peer Support Worker's role to the client as it relates to the larger
- program; respond to questions or concerns

  • Identifying client strengths and needs, helping to develop a care plan to support and maintain client wellness
  • Orient the family members and clients to recovery and talk about the ways to support recovery
  • Support clients in the identification of strengths that people can access to address areas they want to change
  • Support clients to make choices that promote safety, wellness and recovery
  • Invite, encourage, and support selfadvocacy of clients in terms of their clinical relationships
  • Help facilitate supportive groups and support peers to create recovery plans for themselves
  • Establishing and implementing Recovery Plans to guide the mutual work towards client recovery
  • Encouraging clients to identify and work towards educational and vocational goals
  • Providing ongoing supportive counselling, life skills coaching, preemployment coaching and appropriate referrals to enable clients to achieve their goals
  • Consulting, advocating and liaising with community resources and agencies to assist clients in achieving their goals
  • Providing education on substance abuse issues to clients and their families/other supports
  • Referring clients to long term support, as needed
  • Establishing a followup plan for clients prior to discharge
  • Attending and actively participating in team meetings, clientrelated meetings and program meetings as require
