- Assess new patients for complexity of nutritional needs. Refer patients to the unit Dietitian when nutrition assessment meets the requirements for RD follow up;
- Consult with patients and family members regarding patient's preferences;
- Customize meals for patients who need special dietary requirements (dysphagia);
- Ensure patient care plans are up to date;
- Check and correct Care Plans for accuracy of diets;
- Responsible for meal rounds at lunch and dinner and other audits as assigned;
- Request weights for patients as per RD request; advises Dietitian of any concerns;
- Enters patient's preferences into CBORD;
- Records patient's nutritional information in patient's chart.
- Adhere to Unity Health Toronto's documentation standard guidelines;
- Other duties as assigned by the Operations Leader.
- Completed 2-year college Food Service Supervisor diploma and has active CSNM status;
- Three years experience as Food Service Supervisor, or Diet Technician clinical experience preferred;
- Good knowledge of clinical nutrition;
- Very good communication (both written and verbal) skills;
- Knowledge of the CBORD diet office system;
- Very good attendance record;
- Good performance record.
