As a member of the Clinical Support Team at KidsInclusive (KI), the Urgent Response Coordinator support the Urgent Response Service (URS) by providing comprehensive, flexible and timely service including screening of children/ youth with Autism who request the service, providing service navigation for all children/youth referred, and leading the development, monitoring, and evaluation of the Urgent Response Service plan for those who meet the criteria for the service and their families.

The Urgent Response Coordinator provides direct and indirect client care; assists in providing departmental support; documents his/her involvement in client care; maintains communication with appropriate staff members; supports departmental efforts in Continuous Quality Improvement (CQI) activities; participates in education orientation and public relations, as required; maintains a safe work environment; performs other related duties as assigned.

Wherever possible, the Urgent Response Coordinator ensures that clients and families receive complete and accurate information and have the opportunity to ask questions.

They will encourage clients and families to participate in care and decision-making at the level they choose, and collaborate by involving the client and family in the formulation of a plan of care.


PRINCIPLE DUTIES & RESPONSIBILITIES:


Clinical

  • Determine eligibility for URS by: Confirming registration in the OAP (more to be added about relationship to CIRT to quickly confirm status) and supporting the completion of the Step One and Step Two screening in order to determine eligibility for the service.(more to be added)
  • Redirect those not appropriate for URS by: Providing (either directly or linking to) service navigation for those not eligible for URS to support connection to other community services.
  • Support the Development, Monitoring and Updating of the URS plan
  • Provide a coordinated collaborative experience for child and family
  • Collect comprehensive information that supports service delivery at the beginning and throughout the family's participation in urgent response service
  • Act as a liaison in the community at large

Administrative

  • Adhering to all policies and procedures of the Program.
  • Maintaining confidentiality at all times.
  • Fulfilling all requirements of client record keeping in a timely manner.
  • Documenting data required by MCCSS and statistical reporting requirements
  • Implementing evaluation strategies to assess the service impact
  • Ensuring adherence to service protocols
  • Attending staff/ team meetings and participating in discussions regarding service provision.
  • Following all global policies, procedures and systems as outlined in specific manuals.
  • Reporting pertinent feedback to immediate supervisor.
  • Ensuring familiarity with governing legislation and supporting compliance by parents and service providers.
  • Verbally communicating concisely, clearly and in a manner that is appropriate for the listener
  • Accepting other responsibilities as assigned by manager and completing in a timely manner.
  • Provide mentoring/ student placement and shadowing opportunities as requested by management
  • Provide input to the development of department and or hospital quality improvement procedures as requested
  • Participate in ongoing professional development (formal and informal); participate and follow through of Performance Development Plans.
  • Complete elearning modules as required by KHSC/KidsInclusive policies

QUALIFICATIONS:


  • A diploma or degree in social services or sciences; related field may be considered with suitable combination of education and experience
  • Minimum of five years' experience in the Children's Service System and in the community
  • Extensive knowledge of the range of support services available to children and their families
  • Good working knowledge of provincial policies and legislation concerning children and youth.
  • Knowledge of and proven skills in providing service coordination
  • Excellent interpersonal and communication skills demonstrated by the ability to interact, collaborate, and communicate clearly in an effective manner with children, families, and members of the interdisciplinary team
  • Ability to work independently and participate as a team member
  • Ability to assess needs of client and make sound judgments to coordinate activities
  • Excellent organizational skills and ability to multitask
  • Current BCLS at the healthcare provider level
  • Access to a reliable vehicle and a valid driver's license as per KHSC policy
  • Proficiency in speaking and comprehending both English and French (preferred)
  • Proven ability to attend work regularly.
