- Work with clinical network members from across the health system to identify priorities, review current practice, inform and support the development of evidence-based clinical care standards, clinical service delivery models and clinical care pathways with a provincial scope that are patient-centred, equitable, evidence-based and endorsed by subject matter experts
- Engage leaders and critical stakeholders to collaboratively realign system-wide services delivery to better serve the provincial population, drive leading, innovative change across the province, and monitor the effectiveness of implementation over time
- Provide expert advice to provincial integration leads and provincial clinical services leaders to assist them in removing geographical and environmental barriers to identifying and adopting improved models and methods of patient-centred care
- Introduce and consolidate clinical expertise to build provincial models of care or care pathways and the provincial implementation plan
- Participate in building and ensuring the ongoing success of provincial health services networks that govern and influence direct patient care decisions in the service delivery organizations across the province
- Provide technical/clinical input to ongoing updates of Manitoba's Clinical and Preventive Services Plan
- Participate in clinical governance level decision-making teams and influence leaders to adopt, plan, and implement integrated services delivery and inter-organizational partnerships, new province-wide care pathways, standards of practice, tools, and skills to improve patient outcomes with a provincial population perspective
- Collaborate and coordinate with multi-organization/cross-jurisdictional clinical network members to develop material supporting the clinical education and training required for end-users, sites or services in the application and implementation of the clinical change within various clinical environments across the continuum of care
- Collaborate with all other business functions including finance, capital planning, and workforce on integrated teams to ensure effective province-wide planning and integration of services delivery

- 3 years of experience collecting, collating, interpreting, and analyzing clinical literature and information to develop, implement, monitor, and evaluate clinical standards, pathways, and care delivery/service models
- 2 years of experience building and maintaining multi-sectoral partnerships including liaising with provincial and national/federal organizations and governments
- 2 years of experience providing coaching, mentoring clinical professionals, and developing and providing formal training
- 3 years of experience managing limited business resources (financial, human, physical/material, intellectual), organization change, and influencing internal and external stakeholders
- 2 years of experience working as a member of a project team to implement clinical change
- Post-secondary degree in a health profession from an accredited educational institution, a postgraduate degree is preferred
- Active member of and licensed with an associated health regulatory body as appropriate
- Demonstrated mid-level leadership capabilities. The provincial health system in Manitoba has adopted the LEADS in a Caring Environment framework
- Must be able to work safely in a typical computerized office environment with frequent meetings at multiple client sites, and to lift and carry items of up to approximately 10 kg

Shared Health leads the planning and coordinates the integration of patient- centered clinical and preventive health services across Manitoba. The organization also delivers some province-wide health services and supports centralized administrative and business functions for Manitoba health organizations.
Position Overview
The provincial clinical services consultants will:
Working as assigned with a customer-centric mindset in a dynamic team, each position will be assigned to support a Provincial Clinical Service Leader for one or more of the following specialty areas:
This position is responsible for supporting chronic and complex medicine and rehabilitation planning and coordination.
Experience
5 years of experience in clinical supervision and leadership including:
3-5 years of clinical experience in chronic disease management and rehabilitation medicine; in the areas of renal health, diabetes, respiratory and/or cardiovascular care
Education (Degree/Diploma/Certificate)
An equivalent combination of education and experience may be considered
Certification/Licensure/Registration
