Background


ACSA is a nonprofit, multi service agency in Scarborough addressing needs and empowering children, youth, newcomers, homeless, and otherwise underserved communities.

Core programs include housing and outreach programs, food security programs, newcomer services, youth outreach and violence prevention programs, child and family programs, early years' programs.

ACSA works from a Community Development lens and endeavors to work alongside its community to create meaningful programs and services that best meet the needs of people.


Summary of Job Functions


The Street Outreach worker provides mobile outreach in order to reach individuals who are street homeless, and assist these clients to access services and move from the street into shelter and housing.


Duties include:


  • Conduct vanbased outreach and conduct outreach on foot to make contact with individuals staying in parks, bus shelters, squats and ravines, railway yards, living in their vehicles wherever homeless individuals are known to reside
  • Conduct intake and assessments to determine program eligibility and provide information about the program to clients
  • Provide timely response to Concerned Citizen Calls routed through City of Toronto
  • Provide ongoing, housingfocused case management supports to clients including informal counselling, problemsolving around client needs and referrals to community supports
  • Assist clients in attaining and maintaining housing, including helping with housing searches, identification and resolution of barriers to housing, working with landlords, OW, ODSP, and other organizations to assist clients
  • Provide transportation in agency van for homeless clients to housing appointments, shelters, dropins, detox centres or other safe locations, and facilitate their access to listed services
  • Ensure timely maintenance and upkeep of vehicle
  • Provide enhanced services during extreme cold and extreme heat alerts
  • Assist clients in accessing services that can help them to achieve greater selfsufficiency, i

e:
identify document replacement programs, income support programs and medical services

  • Provide clients with housing stabilization support, complete case summaries and submit for case transfers to follow up supports
  • Maintain effective liaison with The City of Toronto, Streets to Homes, and other sources for client referral
  • Engage with local businesses in the catchment area
  • Attend Streets to Homes and agency/team meetings as required
  • Adhere to City of Toronto guidelines as per funder requirements
  • Record daily shift activities in the project log book and relevant data collection tools
  • Maintain uptodate records of client interactions in accordance with agency and City requirements.
  • Maintain data records in SMIS and/or additional data programs as required
Other Duties

  • Actively participate in ACSA dropin programs as assigned or scheduled
  • Actively participate in regular supervision sessions and team meetings
  • Identify and attend training sessions related to this position
  • Submit reports as required, including daily, monthly and semiannual reports to the City.
  • Other duties as assigned.

Qualifications:


  • Diploma/Degree in Social Services/Health or a minimum of one year related experience

Valid G-class driver's license, a clear driver's abstract, and have been insured in Canada and/or the United States for 3+ years

  • Ability to drive an outreach van in all seasons and weather conditions
  • Knowledge of issues of homelessness
  • Excellent interpersonal skills, sound counselling skills, and ability to form relationships with marginalized individuals
  • Sound judgment and good problemsolving skills
  • Knowledge of community resources for homelessness, health, mental health and substance abuse.

Working Conditions


The Street Outreach Services Worker works mainly from an agency van and also in an office setting at the agency's main site.

The Worker will conduct outreach outside in all weather conditions, accessing parks, ravines, remote locations, industrial areas, storage facilities, restaurants and donut shops.

The work requires appropriate clothing and footwear for the current conditions and sites.
