Building a community of hope since 1987, we would like to invite you to join our journey. Siloam Mission is currently seeking for a full-time term Caseworker at the Madison.POSITION DUTIES
Provide case management services to clients, helping them to maximize their ability for self-determination by supporting and empowering them in decision-making and assisting with goal setting, planning and follow through of identified goals;
Assess clients' relevant skills, strengths and development needs;
Act as an advocate for the needs of the clients of the Madison;
Maintain client records in a timely manner;
Monitor, evaluate, and communicate progress of clients with the client themselves, team members, and supervisor on a regular basis;
Provide tenant and eviction prevention services;
Assist residents in determining their strengths and interests and pursuing vocational related activities;
Attend bi-weekly team meetings, attend staff meetings as required;
Participate in all mandatory training;
Other duties by Supervisor.
ESSENTIAL
Excellent human service planning skills and ability to engage a broad spectrum of community members in planning efforts;
Strong background in working with low-income individuals with addition, mental, physical and developmental disabilities;
Strong background in supervision;
Exceptional skills in conflict resolution;
Strong customer service background and skills;
Knowledge of Microsoft office suite;
Excellent interpersonal, communication, and organizational skills, as well as a genuine interest in working with marginalized persons.
PREFERRED
Post-secondary degree in Social Services. Combination of education and experience may be considered;
Experience in case management in the social services field with eminences on assessment, service planning, evaluation, follow up, review and closure;
Experience working with those that have mental health illness and addiction;
Assist, Mental Health First Aide, First Aid and CPR training considered an asset;
Non-Violence crisis intervention training;
Valid driver's license and access to a vehicle in good condition and willing to drive clients to appointments.
ADDITIONAL
Clear Vulnerable Sector Check and Clear Adult Abuse Registry Check will be required.
