A temporary Community Transitional Care Unit (CTCU) containing 32 transitional beds will soon open in the Willow Grove area at Hogarth Riverview Manor.

This is an extension of the Community Support Services program of Sister Leila Greco.

The temporary CTCU will be a place where the physical and emotional needs of clients can be met within a homelike environment.

Members of the CTCU team will provide support for clients who need moderate to high supports with daily living while waiting for space in a community setting.


Qualifications:


Education / Experience:


  • A degree or diploma in one of the social sciences
  • Two (2) years experience working with the elderly within a community care environment
  • Knowledge and experience with gerontology, Alzheimer Disease and related dementias

Skills/Abilities:


  • Ability to communicate effectively and concisely, both orally and in writing
  • Strong facilitation skills along with the ability to deliver presentations information to a variety of audiences
  • Ability to establish rapport with others and to maintain effective working relationships with superiors, coworkers, and customers
  • Strong time management and organizational skills
  • Ability to identify, understand, and meet the requirements of internal and external customers
  • Proficiency with office computer equipment and software including Microsoft Word and Excel
  • Ability to work independently and as part of a team

CONDITIONS OF EMPLOYMENT:


  • Must maintain membership in the Ontario College of Social Workers and Social Service Workers
  • This position shall be conditional upon a satisfactory Police Vulnerable Sector Check (PVSC) where indicated, to ensure the absence of relevant criminal convictions.
