The Opportunity

:


In accordance with Jordan's Principle (JP), providing every Indigenous child with the care that is needed, the Jordan's Principle Service Coordinator will use systems navigation to improve the overall care and service provided to First Nations children and youth with complex care needs.

The JP Coordinator is responsible for providing day-to-day advocacy support to First Nations children and their families as well as service providers, supporting the child and family's navigation of culturally appropriate services.

The position enables integrated and well-coordinated care, across primary, acute and community health and social services.

With a working knowledge of Indigenous health approaches, protocols and culture, the JP Coordinator promotes interdisciplinary collaboration and communication and provides information and referrals to First Nation communities and CSFS service providers.

The JP Systems Navigator also maintains databases and reporting requirements.

Serves as a point of contact for (a) Indigenous children and their families in the CSFS service delivery area (b) health care providers, and (c) First Nations communities and organizations who are requesting support and advocacy directly related to children and youth

Ensures the appropriate and effective sharing of information including identifying and maintaining contact with Indigenous children and families as required for continuity of care, including the use of multiple communication strategies including telehealth

Collaborates with a multi-disciplinary team to provide a timely response when children are in need of care

Develops and documents a coordinated service plan that includes referral, assessment, treatment/admission, discharge (if applicable), transition and ongoing care protocols

Builds strong relationships and partnerships with front line health care and child care workers, and works with a variety of health care and child care professionals, including but not limited to nurses, speech pathologists, dental, occupational therapists. Works with these types of individuals in order to obtain clinical support

Builds strong relationships and partnerships with local Provincial and Federal Government services in the social sector, including, but not limited to, Ministry of Children Family Development, Ministry of Education, the Ministry of Health, Provincial Regional Health Authorities, and Indigenous Services Canada (ISC)

Maintains strong relationships with First Nations health and social sector service providers and organizations, as well as other service providers and other First Nations organizations

Raises awareness of Jordan's Principle risk factors by performing an outreach function i.e., providing health promotion and education to front line health care and child care workers

Regular Travel throughout North Central British Columbia is required.

What You Will Bring to The Role

:

Bachelor's degree in a health or social services program or related discipline or related field of study preferred

:

-
A diploma in a Human Services related field required

:

-
Three (3) years' recent related experience working in health and social services, particularly with Indigenous Peoples.
