KEY FUNCTION:


Provide comprehensive nutritional assessments and care to the Nephrology Program including: Multi-Care Kidney Clinic (MCKC), In-Centre and Satellite Hemodialysis and Home Dialysis patients and program patients admitted to in-patient units at Health Sciences North.


REPORTING:


Under the general direction of the Manager of Satellites, Satellites, Clinics, Home Dialysis & Regional Renal Program Clinical Quality Improvement.


DUTIES:


  • Interpret anthropometric, biochemical, physiological data in relation to nutrition status.
  • Calculate patient's daily macro and micronutrition requirements (e.g. energy, protein, vitamins, minerals, fluids, etc.) based on available data.
  • Provide therapeutic counselling to patients and/or family/caregiver(s) on an individual basis.
  • Monitor patient progress and liaise with the multidisciplinary treatment team and other relevant health agencies.
  • Travel to satellite communities to provide clinics up to twice per year.
  • Attend clinical meetings as required.
  • Identify current and potential nutrition related problems and develop and/or revise clinical nutrition policies and procedures.
  • Act as a preceptor to dietetic learners from Northern Ontario Dietetic Internship Program (NODIP) / Northern Ontario School of Medicine (NOSM) University.
  • Provide education and awareness on the nutritional management of medical issues and healthy eating.
  • Develop and maintain an inventory of relevant patient education materials reflecting current scientific evidence and best practice standards.
  • Attend clinical nutrition and other pertinent education opportunities (i.e. conferences, workshops) as required.
  • Determine and align improvement projects with HSN's Strategic Plan; monitor and adjust to achieve goal outcomes.
  • Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
  • Educate and promote health, safety and wellness in the work place.
  • Represent the department or program on various committees and in meetings as required.
  • Perform other duties as required.

QUALIFICATIONs

EDUCATION AND TRAINING:


  • Minimum of a four (4) year Bachelor's Degree in a Nutrition field, from an accredited university.
  • Successful completion of an accredited Dietetic Internship program.
  • Current Certificate of Registration in good standing with the College of Dietitians of Ontario.
  • Ministry of Labour "Worker Health and Safety Awareness in 4 Steps" training certificate is required.

EXPERIENCE:


  • Minimum of two (2) years' experience working within a health care environment.
  • Experience in the field of Nephrology is preferred.

KNOWLEDGE/SKILLS/ABILITIES:


  • Demonstrated knowledge in nutrition related to Nephrology.
  • Demonstrated ability to work in an interdisciplinary setting and as a member of an interdisciplinary team.
  • Demonstrated training, experience or utilization of lean methodology for process improvement.
  • Demonstrated ability to independently identify issues, plan improvements, measure success and continue improvement.
  • Demonstrated excellent computer skills with proficiency in Microsoft Office software (e.g. Word, Excel, Power Point and Outlook) and patient information systems.
  • Demonstrated superior interpersonal and communication skills, both written and verbal.
  • Demonstrated effective time management and organizational skills with the ability to organize and prioritize as required.
  • Demonstrated discretion and maturity when handling confidential information.
  • Demonstrated commitment to the safety of coworkers and patients.

PERSONAL SUITABILITY:


  • Successful Vulnerable Sector Check is required.
  • Valid driver's license with access to reliable vehicle.
  • Willingness to travel occasionally within Northern Ontario.
  • Demonstrated commitment to ongoing professional development.
  • Demonstrated professionalism in dealing with confidential and sensitive issues.

  • Ability to meet the physical and sensory demands of the job.
