Salary $ $35.430 per hour

DUTIES AND RESPONSIBILITES:
Staff will support individuals with a Developmental Disability and mental health needs and /or challenging behaviours.

Staff will assist with service to individuals, their families or caregivers, and work closely and collaborate with community mental health services to create and maintain effective pathways to specialized services.

The Community Mental Health Worker (CMHW) can provide community based, intensive case management to individuals with mental health concerns.
Support and Advocacy - including assistance in meeting basic needs (i.e. housing, financial), life skills development, crisis intervention (coping skills and safety planning), and referrals/advocacy/coordination of supports

Information and Education - including crisis prevention and management, symptom and medication management, mental health information and education, system navigation.


Minimum Education and Experience:
" Diploma or degree from an accredited College or University in the Human Services field.
" Three years related experience in a human services field.


Knowledge:
" Comprehensive knowledge of developmental services
" Knowledge of major mental illnesses including; signs/symptoms, medication management etc., addictions and sexual abuse issues; trauma informed.
" Understanding of Mental Health Act and confidentiality, principles of client centered relationships. Thorough knowledge of key functions of case management including, local special resources and access to services.
" Understanding of intake function including assessment, decision making, service plans, crisis intervention and coping/stress management strategies.
" Understanding of methods of evaluating progress.

" Strong assessment skills and effective service plan development.
" Ability to engage and build rapport with individuals from a variety of cultural and social-economic backgrounds.
" Effective client advocacy skills.
" Ability to handle multi layered and complex client dynamics and maintain healthy boundaries.
" Client centered and recovery focused.
" Facilitate linkages to internal and external resources.
" Demonstrated ability to work effectively within a team and independently with mínimal supervision, strong time management skills.
" Create a safe, comfortable environment for clients to work within.
" Demonstrated reliability, punctuality and attendance.
" Excellent communication skills, verbal and written.
" Professional, positive attitude.

" Demonstrated understanding of and commitment to Lakeridge Health's comprehensive safety programs and practices including staff, patient and environmental safety and participation in regular in-services in this area; and;

CONDITIONS OF EMPLOYMENT:


The terms and conditions of employment are in accordance with the Collective Agreement between the Hospital and
OPSEULocal 348.


Successful External applicants must provide a satisfactory Criminal Investigation Reference for the vulnerable sector, dated within the past six (6) months.
