The Role:


Reporting to the Day Program Manager, you will be responsible for effectively managing the overall day-to-day operation and coordination of the Adult Day Program (ADP) including supervision of staff, students and volunteers and providing exceptional customer service.


Primary focus of this role is on the completion of intake and routine assessments with the community RAI CHA instrument, care and program planning, discharge and/or transfer to a higher level of care, as an integral part of the continuum of senior care within the CHATS Wellness Framework and direct liaison with Home and Community Care.


What you bring to the role:

  • Degree or Diploma in Recreation, Gerontology or a related field, combined with 3 years' experience in a hospital or community health setting.
  • Program coordination and case management experience required with an understanding of the principles of recreation/therapeutic programming.
  • Volunteer and Human Resources supervisory and team building experience required.
  • Demonstrated success in developing, implementing, and evaluating services and programs.
  • Excellent communication and interpersonal skills
  • Demonstrated ability to work in a multidisciplinary team.
  • Knowledge of medication management practices
  • Knowledge of senior related health conditions
  • Knowledge of budget and fiscal accountability, quality reporting, and monitoring
  • Current CPR and First Aid certificate.
  • Successful completion of the Food Handlers Course.
  • A vehicle and valid Ontario Driver's License required.
  • Certification in Functional Fitness an asset.
  • Fluency in both official languages (English & French), or other language(s) an asset.
