*The EWPCN Central Office is located inside Meadowlark Health &

Shopping Centre:
this is a decentralized position that requires you to meet with various medical clinics throughout the city.

As you may visit multiple clinics within a workday, a valid driver's license and access to a vehicle is required.

We have implemented a blended arrangement to allow you to work from home and our central location/clinic.

Objectives:
The Edmonton West Primary Care Network believes the best primary care is delivered in the Patient's Medical Home.

Practice Improvement Assistants support the EWPCN clinical operations team to enhance the quality of care provided in the Patient's Medical Home.

Practice Improvement Assistants support the optimization of interdisciplinary team-based care and continuous quality improvement by assisting with integration of evidence-based practice into clinical practice and optimizing the electronic medical record (EMR).

Practice Improvement Assistants impact patient outcomes by supporting physicians and their teams to use data to make informed decisions and provide high quality patient care.


ABOUT THE EDMONTON WEST PRIMARY CARE NETWORK:

With a mission to support family physicians in improving primary care through relationships, collaboration and innovation, Primary Care Networks (PCN) are groups of family doctors who work with other health professionals to coordinate the comprehensive delivery of primary care services for their patients.

Our team of health professionals include Nurses, Dieticians, Pharmacists, Behavioural Health Consultants, Social Workers and Exercise Specialists who collaborate to provide comprehensive primary care services for the community.


Qualifications:


  • Medical Office Assistant Diploma, or other healthcare related diploma or degree (e.g. Health Information Management Diploma, data analytics)
  • Medical terminology is an asset
  • 2+ years of experience working in a health care environment, primary care experience is an asset
  • Experience working with Electronic Medical Records (EMR) (2 or more different EMRs would be beneficial)
  • Experience or formal training in quality improvement (i.e. Lean Six Sigma Yellowbelt or Greenbelt), Business Analysis, and/or Data Analytics is a strong asset

Knowledge, Skills and Abilities:


  • A valid Class 5 Driver's License and/or reliable transportation is required for travel between clinics or worksites.
  • All staff require a satisfactory criminal record check, and patientfacing clinical positions also require a satisfactory vulnerable sector check.
  • Model high levels of integrity and professional behaviour.
  • Ability to communicate orally or in writing effectively in the English language.
  • Ability to act responsibly and be accountable for actions.
  • Demonstrated ability to work professionally, effectively and independently in a complex and dynamic environment with mínimal supervision.
  • Proficient knowledge of Microsoft Office Suite with the ability to learn and use other technology platforms.
  • Critical thinking and creativity.
  • Logical and methodical approach to problem solving.
  • Good personal organizational and multitasking skills.
  • Maintain absolute confidentiality in accordance with the Health Information Act and EWPCN policies on privacy.
  • Use good judgment to navigate difficult scenarios appropriately and report concerns in a timely manner.
  • Willingness to learn new skills.

Key Performance Indicators:


  • Development of positive relationships with team members, clinics and physicians in portfolio including partnership with respective interdisciplinary clinicians and Managers, Clinical Services
  • Achieving results on quality improvement indicators specific to the clinic identified in quality improvement plan
  • Supporting clinical excellence through implementation of best practices related to panel management and EMR; supporting scale and spread of QI projects internally and across EWPCN clinic.
  • Approximately 80% of working hours are spent in clinic with the remainder working at the EWPCN Central Office.

Primary Duties and Responsibilities:


Collaboration:


  • Development of professional, trusting relationships with all stakeholders (including physicians, clinic staff, Managers) supported by regular and timely engagement.
  • Capacity building related to patient's medical home enhancement

EMR Expertise:


  • Generate EMR reports to assist the interdisciplinary team with identification of patients and opportunities for screening and care of specific conditions
  • Understand clinic EMR capabilities to assess what's working, not working, and lead clinic EMR optimization
  • Develop and implement processes to ensure EMR data is accurate and up to date
