Geriatric Resource Clinician

Permanent Full-Time

Wage Grid:
In accordance with professional designation of successful applicant

The Geriatric Resource Clinician (GRC) works collaboratively at the patient/caregiver level, facility level, and systems level to facilitate delivery of best practice care for geriatric patients.

At the patient/caregiver level, they are particularly involved with patients who are complex and presenting with one or more geriatric syndromes that are limiting their functional potential.

At the facility and systems levels, GRCs advocate for and participate in initiatives that are based on best practices in geriatric care to improve service delivery.

QUALIFICATIONS:

Registered Nurse or Allied Health Professional with current certificate of competence with their respective professional college
Formal training in clinical geriatrics including completing CGAs at the bedside
Demonstrated ability to manage time, prioritize, collaborate with other health system partners, remain positive during times of stress, and lead a team
Previous experience in geriatrics, gerontology, rehabilitation, adult education, change management, leading/coaching/mentoring would be considered an asset
35 years of frontline experience in a hospital/rehabilitation setting
Evidence of previous success leading or participating in project management
COVID19 Vaccination required to work in our facility (2 Doses)
