One Full-Time Contract Position to March 31, 2025 (with possibility of extension or becoming permanent)


The Transitional Aged Youth (TAY) Coordinator is responsible for the provision of case management and co-ordination for youth with developmental disabilities in York Region who have been identified as children in extended society care or who are in receipt of Complex Special Needs funding and require a coordinated transition to adult developmental services.

The Transitional Aged Youth Coordinator is also responsible for the compilation and analysis of planning data related to developmental services as well as the provision of case planning co-ordination and advocacy services for service users identified as community pressures.


The Transitional Aged Youth Coordinator will facilitate case resolution for complex transitional aged youth cases on behalf of the Developmental Services team.

In addition, the Transitional Age Youth Coordinator will participate in the development of community resources and creative strategies to address service system issues and engage in public education and awareness.
