What you will do


As a Case Manager you will support highly vulnerable newcomer, GAR, BVOR and PSR clients/ families during their settlement process by providing a wrap-around client-centered case management services (in person and/or remote).

Case manager is responsible for providing one-on-one and/or family case management, support and group services to newcomer population who struggle through multiple barriers during their settlement process, with the goal of building and enhancing their capacity towards self-independence and self-reliance.

Through outreach you will connect with eligible newcomer clients with complex needs and multiple barriers that prevent them from successfully settling in Canada.

The services that you will be offering to the client include information, orientation and appropriate referrals to community and government services all done following a wraparound model where services are brought to the newcomer with the newcomer being at the center in a supportive environment and you becoming their bridge to services.

You will conduct a needs assessment to identify client's needs and create a wrap-around service/settlement plan, act as an advocate in helping newcomers access services including education, health care, legal systems and support with filling out forms.

You will provide case conferencing, supported access and transition support to external services where needed. Part of your job will also be to ensure continuous case monitoring and follow up. You will enter information into government/ department databases and prepare monthly narrative and statistical reports for the Program Lead. You might be asked to provide life skills, literacy and essential skills group or individual information session. Additionally you will connect with community partners and explore effective and efficient service sharing. As always, there might be other duties as assigned


What you will bring.


Ideally, you will have a completion of a university or college degree (social work and/or settlement background strongly preferred for this position) or equivalent.

You will also possess the Wrap-Around Canada certification. 1-2 years of relevant work experience in the field of community service and immigrant settlement.

You will have excellent computer knowledge of Microsoft Office and database work, excellent oral and written communication skills, keep abreast with Immigration legislation/policies affecting refugees and immigrants, mediation and conflict resolution.

You will work directly with client information so previous experience working with confidential data and client information is strongly preferred.

Your native knowledge of a second language would be an asset (esp. Arabic, Amharic, Tigrinya or Spanish). We also require Police Vulnerable Sector Records check.
