We are seeking a Home and Community Care Coordinator to support our clients and staff.

Duties and Responsibilities:

Include, but are not limited to:


  • Client intake, creating care plans, and initiating a client/caregiver matched schedule
  • Ensuring accurate client information, service plans, and schedules
  • Achieving and exceeding client satisfaction
  • Working closely with caregivers and team to investigate issues and ensure prompt follow up
  • Managing caregiving performance in collaboration with the Director of Care

Qualifications/Requirements:


  • Current registration with the College of Nurses
  • In-Depth experience in Home and Community Care
  • Experience in scheduling software such as Alayacare, Procura, or Goldcare
  • Be available to be on call (paid) to provide telephone support to clients and caregivers after hours on a rotating basis (approximately 1 week/weekend per 8 week rotation)
  • Valid drivers license and vehicle (travel within community)
  • Negative TB Screening
  • Valid Vulnerable Sectors Police Check
  • Covid Vaccination Certificate
  • Current CPR/First Aid
