TRANSITIONAL SUPPORT PROFESSIONAL

Currently hiring a Full-time 40hrs/week Night Awake position in Pickering, Ontario


The Transitional Support Professional provides critical services to the people supported by Karis Disability Services Designated Treatment Programs and works to ensure that people belong to communities in which their God-given gifts are valued and respected.

The Transitional Support Professional seeks to promote Karis Disability Services' Service Principles by enhancing relationships, promoting citizenship, valuing people's gifts, recognizing self-determination, encouraging growth and development, and recognizing the need for holistic supports in a manner that considers each person's intrinsic value.


The Transitional Support Professional assists with activities of daily living and promotes life development as outlined in person centered plans.

This is done through collaboration with their team, families, advocates, friends, and community partners. As part of the clinical duties, the Transitional Support Professional may be required to provide reports on clinical interventions.

They liaise with clinical professionals, collect appropriate data, and actively carry out appropriately prescribed programs & treatments to address the clinical, behavioural and/or mental health needs of people supported by Christian Horizons.

They completes administrative duties and responsibilities as required or assigned.

Requirements:


  • Satisfactory Vulnerable Sector Police Check
  • Education & Experience

i) Experience:

  • Minimum one year in developmental services

AND

ii) Education (one of the following):

  • Developmental Services Worker Diploma (DSW) or
Apprenticeship, or DSW Apprenticeship actively in progress

  • Child and Youth Care Diploma (CYC) or Apprenticeship or
  • Diploma or Degree in a Related Human Services field such as: Community Mental Health, Behavioural Sciences, Behavioural Science Technology, Behaviour Analysis/technologist, Police Foundations Diploma, Social Service Worker, Therapeutic Recreation, Recreation Therapy, Autism and Behavioural Science, Educational Support, Social Work, Psychology, or Honours Applied Science in Family and Community Social Services

And

  • Minimum one year in developmental services

OR

Experience:
Minimum 5 years experience supporting people with behaviour support plans

  • Access to reliable transportation

Pay:
$24.83-$29.44 per hour
