West Park Healthcare Centre helps patients get their lives back by providing specialized rehabilitative and complex care after a life‐altering illness or injury such as lung disease, amputation, stroke, and traumatic musculoskeletal injuries.


West Park is building a new hospital and transforming its 27-acre site into an integrated campus of care, enabling the hospital to evolve its rehabilitative programs to meet Ontario's future healthcare needs.


Qualifications:


  • Grade 12, or equivalent education
  • Personal Support Worker Certificate received from an Ontario College of Applied Arts and Technology program or received from an Accredited PSW Educational Program
  • BCLS certificate preferred
  • At least one year of full time equivalent work experience within the past three years preferred
  • Strong verbal and written communication skills and the ability to deal with the public in a courteous manner
  • Ability to function as a team member in an interprofessional collaborative environment
  • Ability to accept direction and work independently
  • Basic computer skills related to Microsoft Office (e.g., Word), Internet, and Outlook Express.
  • Satisfactory work performance in present position.
  • Evidence of satisfactory attendance record
  • An employee's disciplinary record will be reviewed as part of the assessment of qualifications
  • Ability to work in a manner that exemplifies the centre's core values and helps patients_ get their life back_
  • Should be aware of conditions and measures that may affect the safety of patients, residents, students, employees, volunteers, physicians and visitors

Accountabilities:


  • Follows the KARDEX in meeting patient's care needs and preferences related to communication, meal assistance, transferring, ambulation/mobility, toileting/continence care, hygiene, and grooming
  • Follows the individualized care plans created by the healthcare team in meeting patient's care needs
  • Reports to the Registered Nurse in a timely, objective manner without interpretation of any changes in patient condition and any patient and/or family concerns
  • Assists in meeting the psychosocial needs of patients in collaboration with the healthcare team
  • Supports the effective operations of patient care areas
  • Supports the effective transfer of information about patients by relaying information to the appropriate healthcare professional in a timely, objective, and comprehensive manner.
  • Documents according to hospital policy
  • Operates mechanical lifts, power wheelchairs, and assistive devices as directed. Ensures equipment is operational and intact. Follows procedures and advises appropriate staff of equipment concerns or need for repairs
  • Performs activities required to assist the healthcare team in providing a safe, clean and comfortable environment for patients
