We are currently recruiting for a permanent, full time (37.5 hours per week) Social Worker with the Paediatric Acquired Brain Injury Community Outreach Program (PABICOP) in Family and Community Services.

The designated office location for this position will be London.


This dynamic role provides support to children and youth (0-18 years old) in Elgin, Middlesex, Oxford, Huron, and Perth counties and involves working with a variety of fellow clinicians, physicians, educators, and parents to help children and youth achieve meaningful outcomes after an acquired brain injury.

Regular travel will be required.


Responsibilities:


  • Assess and provide psychosocial support and guidance to children and youth with an acquired brain injury and their families within a holistic and comprehensive framework using a variety of methods including facetoface meetings, telephone, brief counselling and virtualcounselling;
  • Prepare and support children and families in the transitions from hospital to home, community and return to school;
  • Provide education for clients, families and others regarding acquired brain injury including concussion;
  • Assist families to identify, plan and coordinate services for their child;
  • Connect families to supports and services in their communities and help them navigate service systems;
  • Build relationships within the community to develop a foundation for success for PABICOP clients and their families;
  • Help the family with practical considerations such as identifying financial supports, completing paperwork, solving issues such as transportation, or finding informal supports in their community;
  • Participate in service development and evaluation;
  • Provide community and Centrebased intervention within the LifeNeeds Model of Service
  • Participate in and /or integrate and share current research/literature on acquired brain injury, family and community development;
  • Participate in multidisciplinary PABICOP Clinics and PABICOP weekly Rounds at TVCC;
  • Supervision of students may be required.

Qualifications:


  • Master's Degree in Social Work;
  • Current registration with the Ontario College of Social Workers and Social Service Workers;
  • Experience with children, youth and families;
  • Experience with or a demonstrated understanding of acquired brain injury, including current best practice for concussion management;
  • Experience working with teams such as school board staff, medical teams, and/or specialty clinic teams is considered and asset;
  • Knowledge of current models of service provision and clinical expertise relating to children and youth with disabilities and their families;
  • Previous clinical experience and skill with assessment, family system analysis, strengthsbased interventions and individual brief counselling;
  • Knowledge of children's services including rehabilitation, mental health, child care and education;
  • Strengths in interpersonal relations, communication, and problem solving, with the ability to support a multidisciplinary team in stressful family and community work environments;
  • Current knowledge of best practices and literature related to disability, family and community development and the Life Needs Model;
  • Computer proficiency including Microsoft Office 365 and videoconferencing software (e.g., Teams);
  • Valid Ontario driver's license and access to a car are required;
  • Current First Aid and CPR certification.
