Role Overview

Under the general supervision of the Project Manager, Convening Project with PHAC, the Health Equity and Evaluation Officer performs research and planning functions for a hybrid Diabetes Canada and Public Health Agency Canada (PHAC) project.

This person will foster relationships to expand equity and inclusion through collaborative planning, customizing, assessing, coordinating, and facilitating ongoing conversations around health equity and social determinants of health through a diabetes lens.

The Health Equity and Evaluation Officer will assist in evaluating and summarizing health disparities, health equity infrastructure needs, and capacity needs to develop an overview by region and/or province.

They will contribute to mobilizing efforts and provide collective knowledge to partners around social determinants of health and health equity.

While the Officer organizes and supports roundtable meetings, they will also collaborate with partners to promote and evaluate health equity in communities.

The Officer will engage with diverse community partners in developing, implementing, evaluating programs, and services that affect the health of communities' Canada wide.


Contract Length:
This role is a 12-month contract.


This role includes, but not limited to:

  • Work with health researchers and clinicians to prepare assessments, analyze the data and use results to identify key focus areas
  • Help with the creation and monitoring of evaluation matrix for projects
  • Conduct and summarize literature reviews to share with both internal and external stakeholders
  • Summarize and prepare evaluation results and make them accessible to partners, stakeholders and general community.
  • Organize and convene partners and advocates to support the achievement of equitable outcomes among diverse communities.
  • Establish a mechanism for DC's Mission teams to gather feedback on diabetes education and resource needs within the community
  • Plan and implement meetings to address health equity and social determinants of health & facilitate group discussion.
  • Establish and maintain effective working relationships with various partnering agencies such as medical clinics and centres, communitybased organizations, worksites, government officials, social service agencies etc.
  • Conduct research to identify best practices to assure culturally responsive relationship building with diverse communities.
  • Maintain an electronic repository of equity tools, promising practices, and culturally proficient community engagement strategies.
  • Contribute to the identification and cataloguing for review of strengths, weakness, challenges, and opportunities of select Canadian diabetes programs
  • Prepare reports and grey papers, plans of action, public presentations as well as both written and oral communication.
  • Organize and support round table meetings
  • 2+ years of relevant experience in program planning and evaluation, community outreach and assessment
  • College or University Degree in public or community health, social work, nursing, environmental health
  • Considerable knowledge of public health and the social and economic factors influencing individual and community health.
  • Strategic planning as necessary to promote equitable outcomes.
  • Strong written and verbal communication skills
  • Requires the ability to synthesize data and other information to draft reports and grey papers
  • Able to work well under pressure, manage multiple tasks simultaneously, work well in team situations, meet deadlines, and be able to work independently on a daytoday basis.
  • Excellent proofreading skills
  • French is an asset
