    (Full Time Permanent):

CMHA Toronto proudly provides services to over 10,000 clients per year, in 31 programs at 13 locations across the City of Toronto.

RESPONSIBILITIES

  • Outreach and assessment
  • Orientation to program and agency
  • Clientcentered engagement, service provision and interventions
  • Skill development and goal planning
  • Providing resources, information and advocacy
  • Maintaining a trusting, respectful and collaborative working relationship
  • Monitoring, evaluation and followup
  • Maintaining up to date files.
  • Site based program development and program facilitation with TRHP
  • Some housing responsibilities including coordinating unit maintenance/inspections, assisting with weekly food bank, meal preparation for programming.
  • Supporting clients with ADLs/IADLS.
  • Attending client appointments when required
  • Some support of forensic clients, specifically medication monitoring and urine drug screen (UDS) collection.

SKILLS YOU BRING

  • Bachelors level education in Social Work/Social Science.
  • 3 years' experience in community mental health.
  • Understanding of the Strengths
- based model.

  • Knowledge of Trauma Informed Care service delivery.
  • Excellent skills in providing case management support.
  • Work experience and knowledge of concurrent disorders, homelessness, and the criminal justice system as it relates to mental health.
  • Periodic unit inspection to ensure health and safety conditions are being kept.
  • Responding to crisis calls and provide followup support.
  • Excellent communication, listening, and interpersonal skills.
  • Ability to work effectively in a team as well as independently
  • Strong organizational skills.
  • Ability to demonstrate a nonhomophobic and antiracism approach.
  • Ability to communicate in a second language is an asset.
  • Primarily day hours with occasional evenings.

$50,578
- $62,780**
Hours: Monday-Friday; 12:00pm-8:00pm
