Position Description:


As part of the Community Network of Specialized Care Toronto Region team, the Behaviour Facilitator's primary focus is behaviour support for the person versus the system and will relate to serving people as part of capacity building within and between systems.

Reporting to the Manager, Community Network of Specialized Care (CNSC) Toronto Region, the Behaviour Facilitator contributes to the excellence of care for adults with developmental disabilities who have high support and complex care needs and meet the mandate of the CNSC.


Main Responsibilities:


May include, but are not limited to:

  • Provide Applied Behaviour Analysis (ABA) assessment, treatment, consultation, workshops, education, and resource support to complex clients who meet the High Support and Complex Care Needs criteria and their families.
  • Provide support to professionals and various organizations, including schools, longterm care facilities, hospitals, and residential programs in both the developmental and mental health sector with the goal of reducing problem behaviours and enhancing skills.
  • Provide information to clients, caregivers, service providers and staff on the availability and links to specialized supports in the community, including Behaviour Therapy, Occupational Therapy, and Psychiatry.
  • Provide environmental and safety assessments to ensure that the individual is being supported and the family and care providers (including health care, developmental and CNSC staff) are safe. Ensure that there is an appropriate safety plan developed for situations requiring one.
  • Facilitate the acquisition of required behaviour supports and services.
  • Coordinate support to enhance existing behaviour resources and services and liaise with different sector partners.
  • Build partnerships with stakeholders such as hospitals and community agencies.
  • Complete behavioural support and transition plans for clients who are precariously housed or who will require timely intervention.
  • Other duties as assigned.

Qualifications:


  • Bachelor's degree in Psychology or equivalent work experience, with specific expertise in Behavioural Analysis.
  • 5 years' experience working as a Senior Behaviour Therapist.
  • BCBA
  • Comprehensive theoretical and practical knowledge of ABA principles.
  • Specialized knowledge required for conducting functional assessments.
  • Demonstrated, broadbased knowledge and experience working with adults with a dual diagnosis and complexities and a consultative model of service delivery.
  • Knowledge of communitybased services and systems.
  • Experience with assessing mediator abilities/challenges and modifying strategies to meet their needs.
  • Demonstrated experience working with adults with complex needs and participating on an interdisciplinary team and multicultural environments.
  • Ability to work flexible hours.
  • Knowledge of regional resources and service systems.
  • Proficient with Microsoft Office 36
  • Fluency in French is considered an asset.
  • A satisfactory Police Records Check (Vulnerable Sector Screening), and proof of being fully vaccinated for Covid19.
