    Résumé des fonctions

    L'infirmière gestionnaire de cas est responsable de la gestion de cas spécifiques qui lui sont attribués pour assurer une coordination et un soutien durant le parcours du patient. À ce poste, vous serez responsable de la gestion globale des dossiers de patient et agirez à titre de premier point de contact pour les fournisseurs de soins de santé attitrés. En outre, vous veillerez à stimuler une expérience client et patient positive dans un environnement de travail rapide et dynamique.

    Partie A – Responsabilités spécifiques

    • Aider le médecin prescripteur à terminer l'inscription du patient; recueillir les renseignements appropriés pour remplir les formulaires requis (assurance privée ou régime provincial) d'une manière concise, organisée et professionnelle
    • Faciliter les communications requises avec les médecins, le personnel des programmes de soutien aux patients et les autres fournisseurs de soins de santé impliqués dans le cercle de soins des patients
    • Collaborer avec d'autres membres de l'équipe à divers postes pour coordonner le parcours du patient
    • Coordonner la soumission de documents et les dossiers auprès des compagnies d'assurances, des cabinets de médecin et d'autres professionnels de la santé pour accompagner le patient dans son parcours
    • Fournir une orientation pédagogique aux patients et aux médecins sur l'administration de produits
    • Agir à titre de ressource et liaison centrales pour les parties prenantes des programmes, notamment les patients, les fournisseurs de soins de santé, les cliniques et les pharmacies
    • Veiller à ce que les dossiers de patient soient exacts et tenus à jour, conformément aux exigences de déclaration liées aux programmes
    • Identifier les effets indésirables et en faire rapport
    • Fournir des réponses aux demandes d'information générale soumises par téléphone (appels entrants et sortants) ou par une plateforme en ligne à propos des services offerts dans le cadre des programmes, de problèmes de santé et d'autres questions liées aux produits, conformément aux lignes directrices des programmes
    • Comprendre le cadre général du remboursement et s'assurer que les bons documents sont soumis aux assureurs pour obtenir un résultat positif dans un délai opportun
    • Établir des relations avec des clients clés dans la zone de responsabilité afin que les exigences opérationnelles et la gestion de portefeuilles soient clairement établies
    • Mener des visites des lieux pour appuyer les fournisseurs de soins de santé et leur distribuer les documents basés sur les exigences des programmes
    • En mesure de travailler un éventail de quarts, y compris le jour, le soir, la fin de semaine et lors de congés fériés, au besoin
    • Assumer d'autres fonctions au besoin

    • Être une infirmière autorisée (IA) ou une infirmière auxiliaire autorisée (IAA) en exercice, membre en règle de l'ordre professionnel approprié

    Job Summary:

    This role is responsible for the management of specific cases as assigned for the coordination and support through the patient's journey. The Nurse Case Manager (NCM) is responsible for the overall management of patient cases and acts as a primary point of contact to assigned health care providers (Hcp). Additionally, the NCM is responsible for driving positive customer and patient experience in a fast-paced work environment.

    Section A – Specific Responsibilities:

    • Assist the prescribing physician to complete patient enrolment; gather appropriate information to complete the necessary forms (private and/or provincial insurance) in a concise, organized, and professional manner
    • Facilitate any required communication with physicians, Patient Support Programs or other Hcp involved in the patients' circle of care
    • Work collaboratively with other team members in various roles to coordinate the patient journey
    • Coordinate document submissions and cases with insurance companies, physician' offices and healthcare professionals in order to assist in the patient journey
    • Provide educational guidance to patients and physicians on product's administration.
    • Act as a central resource liaison for program stakeholders, which include patients, Hcps, clinics and pharmacies
    • Ensure patient records are accurate and maintained to meet program reporting requirements.
    • Identify and report adverse events
    • Provide answers to general inquiry calls submitted via telephone(inbound/outbound) or web -based platforms on program services, disease state or product related questions, based on program guidelines.
    • Understand the wide reimbursement landscape and ensure correct documents are submitted to insurers for a positive outcome in a timely manner
    • Build relationships with key customers within area of responsibility to ensure that business requirements/ portfolio management is clearly established
    • Conduct field visits to support Hcps with paperwork based on program requirements
    • Able to work a variety of shifts including days, evenings, weekends and holidays as required
    • Other duties as assigned

    Section B – Key Requirements: (Knowledge/Skills/Abilities/Experiences)

    • Practicing Registered Nurse (RN), Licensed Practical Nurse (LPN/ RPN). Must be a member in good standing of respective regulatory college

    As part of Total Rewards, we are proud to offer a competitive compensation package at McKesson. This is determined by several factors, including performance, experience and skills, equity, regular job market evaluations, and geographical markets. In addition to base pay, other compensation, such as an annual bonus or long-term incentive opportunities may be offered.

    Our Base Pay Range for this position

    $49,800 - $83,000
