- Reviews all referrals for Palliative Care Services from clients, families, and PMH staff including program eligibility and program intake.
- Collaborates with the Palliative Care Team and interdisciplinary team (nursing, Home Care, Physicians, Nurse Practitioners, Patient Navigators, Pharmacy, etc.) on required service needs for clients from referral.
- Using clinical assessment skills and clinical resource tools performs clinical assessment on client.
- Using communication skills completes intake interview with client and family (as applicable) utilizing tools focusing on symptomatic concerns.
- Develops an active plan of care including the coordination and planning of interventions in the delivery of care to meet the clinical needs of the client.
- Actively engages interdisciplinary staff for the planning and care delivery of clients.
- Evaluates client care on a regular basis including revising care goals in the alternation of the plan of care to meet the needs of the client.
- Manages client cases prioritizing as required.
- Acts as a clinical resource to nursing and physicians on palliative care principles, philosophy and services.
- Provides clinical expertise for nursing and physicians as it relates to pain and symptoms management of clients.
- Provides direct nursing care to clients and families as necessary.
- Provides psychosocial and spiritual supports to clients and family throughout the course of client illness and into bereavement.
- Participates in the development and review of palliative care polices including policies that pertain to the philosophy and principles of palliative care in other programs and services.
- Assists in the development and delivery of educational and training materials for staff, volunteers and communities on the norms and standards of practice for palliative care including pain and symptom management clinical best practices.
- Advocates on behalf of the client and family to ensure quality end of life care.
- Provides client and family teaching on end of life care including but not limited to advanced care planning, various program available and support and bereavement.
- Promotes the understanding and philosophies and principles of palliative care for caregivers and communities.
- Assists in the completion and submission of application for provincial palliative care programs; Drug Access Program, Oxygen Access.
- Participates in activities or meetings of Provincial and Regional Palliative Care Committees.
- Participates with other provincial RHA staff in the coordination and delivery of community camps and services.
- Documents according to all regional policies and regulatory standards of practices and documentation principles.
- Completes all required statistical reporting within the designated time periods.
- Sundry duties as assigned.

POSITION SUMMARY
The Palliative Care Clinical Resource Nurse is an integral member of the Palliative Care Team reporting directly to the Regional Manager of Clinical Programs. Working with the interdisciplinary teams of Prairie Mountain Health the Palliative Care Clinical Resource Nurse is resource to staff while working with clients and families requiring end of life care and support in acute care, transitional care, long term care and the community. The Palliative Care Clinical Resource Nurse is responsible for the assessment, planning and delivery of palliative care services to clients and families within Prairie Mountain Health while working collaboratively with volunteers, communities, and other Regional Health Authorities for service delivery. The Palliative Care Clinical Resource Nurse functions within the vision, mission and values of Prairie Mountain Health while adhering to professional standards of practice, code of ethics and Canadian Hospice Palliative Care Association.
