We're looking for a compassionate professional to join our Social Prescribing team—someone who sees the strengths in every senior and is committed to advancing health and wellbeing through inclusive person-centered supports.From nutrition and exercise to social connection and caregiver support we walk alongside seniors as they explore what matters most to them
Responsibilities
• Partnering with seniors to complete intake assessments co-create wellness plans including frailty assessments every 3 months.• Building strong connections with seniors through home visits ongoing support grounded in respect autonomy dignity.
