Job Summary:


Baycrest has been appointed as the health service provider accountable for leading the implementation of the Toronto Central regions' Behaviour Support Action Plan.

A key component of the plan is the expansion of the Behaviour Support Outreach Team ('BSOT'), consisting of interdisciplinary community outreach clinicians, nurses and personal support workers who have behavioural support expertise.

The Behaviour Support Specialist will travel and work in the long term care and community sectors to help with managing responsive behaviours, capacity building, and data collection.

The Behaviour Support Specialist will complete client observations and assessments, recommend appropriate interventions, model strategies and assist with transitions across sectors.


Responsibilities include but are not limited to:

  • Provides comprehensive clinical assessments with a focus on identifying factors contributing to responsive behaviors utilizing a bio-psychosocial framework
  • Collaborates with clients, care partners, and the client's circle of care to develop and disseminate individualized behavior management plans in the community and longterm care sectors
  • Assists staff in the Toronto Central LTC facilities to understand and develop behaviour approaches to providing care to residents requiring behavioural support
  • Collaboratively develops and plans assignments with PSWs, community agencies and Long-Term Care Teams
  • Participates in team meetings and contributes to team development
  • Participates in data collection and reporting on a number of accountability measures on a weekly basis
  • Performs teaching responsibilities, participates in relevant education and research activities, and performs duties consistent with the job classification, as required.
  • Performs departmental responsibilities to contribute to the efficient and effective provision of service
  • Ensures the protection of client rights and acting in an advocacy role for the resident/clients

Qualifications include but are not limited to:

  • Completion of a Master's degree program in a healthcare profession
  • Current registration/licensure in good standing as required by a professional body
  • Minimum three (3) years related experience in geriatrics, communitybased seniors care and behavioural management
  • Experience/familiarity with Resident Assessment Instrument—Home Care (RAI-HC) and behavioural, cognitive, psychosocial, and environmental assessments relating to responsive behaviours
  • Demonstrated expertise and knowledge of working with individuals with various types of dementia, delirium, and those with acute behavioural change
  • Demonstrated competence in the use of standard psychogeriatric assessment tools and behavioural support management approaches
  • Demonstrated excellent clinicalreasoning, critical thinking and problemsolving skills
  • Training in or knowledge of Gentle Persuasive Approach (GPA), P.I.E.C.E.S. U-First, Montessori Approach
  • Effective assessment, intervention, client centered and collaborative care planning skills
  • Experience providing caregiver education, support and counselling relating to caring for a person with responsive behaviours
  • Demonstrated experience working collaboratively with primary care and specialists in the care of older adults with responsive behaviours
  • Ability and willingness to travel in the Toronto central region
  • Proficient computer skills (Microsoft Office Suite) and the use electronic communication devices and software pertaining to risk management and RAI documentation
