- Completes nutrition assessments.
- Develops, monitors and revises nutrition care plans.
- Provides nutrition education.
- Keeps records as per standards established by the region/program.
- Provides nutrition expertise for relevant program-specific procedures.
- Participates in multi-disciplinary team meetings ( Care Conferences, Clinical Rounds, and Discharge Planning).
As a member of the Prairie Mountain Health Facility Based Clinical Dietitian team: - Participates in team meetings, including development of the program action plan, and nutrition care-related policies and procedures.
- Participates in sub-committees, as required.
- Reports on relevant issues from clinical areas worked.
- Shares area of knowledge and expertise with team.
- Prepares statistical and status reports for the Area Manager.
- Sundry duties as assigned.
