    Unit Summary

    The Pharmacy Department at the Juravinski Cancer Centre is highly skilled in managing drug therapy and is deeply committed to providing top-notch oncology pharmaceutical care. Pharmacy staff actively engage in both retail and general oncology settings, with a strong focus on patient-centered health care. By collaborating with primary care teams, we aim to maximize the potential of all pharmacy roles and enhance patient care services.

    Position Summary

    Reporting to the Pharmacy Site Manager at the Juravinski Cancer Centre, the Drug Access Facilitator plays a key role in assisting Patients, Physicians, Pharmacy, Social Work, Nursing, and Finance in accessing and distributing Special Access Program drugs. They are responsible for coordinating ODB or third-party submissions, referring patients to Roswell Park, and managing private patient payments within the Oncology setting. The Facilitator works closely with HHS oncology health professionals to ensure the best possible patient care outcomes through effective drug coordination. The Drug Access Facilitator helps eligible patients access oncology medication by providing information on public and private support and funding options such as ODB, Trillium, industry-sponsored patient assistance programs, compassionate plans, and third-party plans. They maintain detailed records of drug access and patient interventions, provide progress updates to the patient care team, and collaborate with Finance to coordinate private patient payments. Additional responsibilities include coordinating physician SAP applications, oncology assistance programs, and purchase orders for chemotherapy. The Facilitator also assists in patient access to the CCO Free Drug Service and coordinates patient referrals to Roswell Park and LHIN partners when necessary.

    Qualifications

    1. Post Secondary education with hospital-related certification/Degree, Pharmacy experience and Certification preferred
    2. Experience with third party payers and assistance programs
    3. Minimum 3 years of oncology-related experience in an Acute Care hospital
    4. Proficiency with computers including word processing, spreadsheet applications and e-mail systems
    5. Excellent verbal and written communication skills
    6. Strong ability to follow specific instructions accurately
    7. Outstanding interpersonal, communication, teamwork, problem-solving, and organizational skills
    8. Demonstrates initiative and is proactive in issue identification and resolution

    As a condition of employment, proof of full COVID-19 vaccination is required.
    This position is temporary until June 2025.

    Schedule: Monday to Friday; daytime hours.
