    Position Summary

    As a Social Worker, in our MASTRS & OASIS Programs, (programs for older adults), you will be an integral part of a collaborative, inter-disciplinary team focused on delivering seamless services and programs to our older adult clients and customers.

    Hours of Work:
    Permanent, full-time. 35 hours a week. Some evening and weekend work is required.

    What you'll be doing


    • Conduct a comprehensive assessment for the client relevant to biopsychosocial history, diagnosis and ability to make effective use of treatment
    • Integrate the biopsychosocial history into the treatment plan
    • Develop and evaluate treatment plans in collaboration with the clinical team, community supports and families.
    • Facilitate group counselling sessions
    • Provide individual counselling in support of patient's specific needs and goals
    • Ensure continuity of care is maintained through the transfer of information
    • Work with the clinical team to initiate community treatment orders
    • Establish relationships with key community supports
    • Discharge planning
    • Prepare referrals to appropriate community services
    • Document in a timely manner
    • Collaborate with the multidisciplinary team
    What we're looking for


    • Master's Degree in Social Work (MSW) required
    • Registered member of OCSWSSW
    • Working knowledge of the Mental Health Act
    • Demonstrated experience working with addictions and concurrent psychiatric disorders
    • Trained in therapy interventions used in mental health (CBT, DBT, Motivational Interviewing)
    • Knowledge of standardized and non-standardized assessments
    • Experience in program development and delivery
    • Knowledge of local community social services
