- Conduct a comprehensive assessment for the client relevant to biopsychosocial history, diagnosis and ability to make effective use of treatment
- Integrate the biopsychosocial history into the treatment plan
- Develop and evaluate treatment plans in collaboration with the clinical team, community supports and families.
- Facilitate group counselling sessions
- Provide individual counselling in support of patient's specific needs and goals
- Ensure continuity of care is maintained through the transfer of information
- Work with the clinical team to initiate community treatment orders
- Establish relationships with key community supports
- Discharge planning
- Prepare referrals to appropriate community services
- Document in a timely manner
- Collaborate with the multidisciplinary team
- Master's Degree in Social Work (MSW) required
- Registered member of OCSWSSW
- Working knowledge of the Mental Health Act
- Demonstrated experience working with addictions and concurrent psychiatric disorders
- Trained in therapy interventions used in mental health (CBT, DBT, Motivational Interviewing)
- Knowledge of standardized and non-standardized assessments
- Experience in program development and delivery
- Knowledge of local community social services
