Description

The Health Promoter will support a program enabling access to social interventions for isolated seniors Responsibilities will include leading patient and community consultations, working with patient partners to inform decision making, and development of community partnerships and pathways to support program objectives.


DUTIES & RESPONSIBILITIES:

Develop and carry out an equity-informed plan for recruiting patient and community partners
Establish a community advisory group to oversee program activities, and build a structure for regular engagement over time
Conduct outreach to socially marginalized seniors, in collaboration with team health providers
Support intake into various programs across the Family Health Team (FHT)
Engage with external Communities of Practice related to social prescribing
Contribute to program planning activities
Lead the creation of a community asset map
Liaise with external organizations, including community social support agencies, legal aid clinics, homeless support agencies, advocacy groups and agencies focused on vulnerable sub-populations to develop referral pathways and supports for patients, with a focus on empowering patients to become change agents
Coordinate and communicate program initiatives across the family health team
Liaise with departmental Anti-Racism Advisory Committee, to ensure partnerships reflect health equity principles
Build organizational knowledge on Social Prescribing

Create educational sessions and communications for physicians (CME) and other FHT staff (CPD), as well as for health provider trainees.

Develop tools to assist in the identification and intervention of social isolation as a health risk
Contribute to primary care settings building capacity to address poverty as a health risk
Working in collaboration with a high-pace equity team to incorporate best practices in critical anti-racism and health equity into care decisions


QUALIFICATIONS:

Graduate degree from a recognized university in health promotion, public health, social science, or a related discipline.
Experience in community outreach, program planning, implementation and evaluation
Familiarity with community consultations and needs assessments
Comprehensive knowledge of the social determinants of health
Understanding of the core values of health promotion, including a grounding in critical anti-racism, social justice, and sustainable development
Demonstrated commitment to and knowledge of community-based health care and resources.
Demonstrated ability to work effectively in a multi-disciplinary team environment.
Ability to work with diverse groups, and/or lived experience of social marginalization
Excellent verbal and written communication skills in English. The ability to speak another language reflecting the diversity of the city of Toronto is an asset.
Experience in building relationships with community partners that are outside the health care system.
Experience in the development of educational materials targeted at health providers and patients.
Experience with data collection, research and quality improvement.
Lived experience of encountering and addressing complex equity, diversity and human rights issues strongly preferred
