The Role and Opportunity:

The Canadian Mental Health Association is looking for a
Case Manager for the
Healthy Families Program.

The Canadian Mental Health Association Toronto Branch is a non-profit community based mental health agency providing a wide range of services across Toronto.

The Healthy Families Program supports caregivers who have children that are being supported by the Children's Aid Society of Toronto (CAS-T) and are struggling with mental health and/or addictions issues.

This team is a multidisciplinary team providing community-based assessment, short-term support and linkages. Referrals come from CAS-T branch and the program is based at the CAS-T office located on Isabella Street,

RESPONSIBILITIES
Reporting to the Program Manager, the Case Manager will work with the interdisciplinary team to support their clients.

They will be required to participate in team meetings and provide regular updates to managers as well as providing education and consultation to CAS workers as required.


The aim of the Case Manager will be to assist in client assessments, skills teaching and care options for the clients.

They will provide supportive counseling, case management and crisis intervention.

This will begin with intake of client information to assess their needs and identifying the supports required, in consultation and collaboration with the client.


The Case Manager will work from a recovery based, trauma and violence informed perspective to build and maintain a trusting, respectful and collaborative working relationship with clients, including client-centered engagement, service provision, interventions, skills development and goal planning.


SKILLS YOU BRING

  • Undergraduate degree or diploma in social services or related field
  • Minimum of three (3) years of experience working with people with mental health and addiction issues in a communitybased setting
  • Knowledge and experience of evidencebased behavioral interventions (CBT/DBT), addictions treatment and approaches to traumainformed care
  • Experience conducting biopsychosocial assessments with a focus on family strengths and crisis intervention skills.
  • Excellent organizational, interpersonal and communication skills
  • Ability to work with diverse communities and demonstrate an antioppressive framework
  • Ability to work independently as well as within a dynamic team environment
  • Valid Ontario Driver's License, access to a vehicle and insurance coverage.
