- Assess patients within an acute rehabilitation setting and provide nursing interventions.
- Collaborate with patients and their families to develop care plans.
- Participate in care and discharge planning processes.
- Coordinate nursing care, set priorities, and delegate tasks.
- Collaborate with the interprofessional care team.
- Educate patients and families on health topics.
- Document assessments and interventions.
