The Falls Prevention Coordinator supports the program development and implementation of Circle of Care's Fall Prevention Programs in community locations.

The Falls Prevention Coordinator will facilitate workshops to teach proper exercise techniques, principles, procedures, share Falls Prevention education and lead wellness, health and education programs.


We are looking for:
a part-time, permanent Falls Prevention Coordinator


Salary:
Commensurate with experience


Hours of Work: 24 hours per week


Reports to:
Supervisor, Exercise and Falls


Responsibilities:


  • Develops, coordinate and evaluate the effectiveness of the agency's Falls Prevention program and initiatives.
  • Coordinate and lead a 12week Fall Prevention program including facilitating virtual and inperson presentations and workshops.
  • Develop and prepare education and instructional materials for workshops and presentations.
  • Conduct assessments on individuals at the beginning and end of the Falls Prevention program and inputting scores and data into client files.
  • Collect and maintain up to date client's data entry, statistical tracking, and compile monthly reports.
  • Provide assessment support to referral sources and community partners
  • Build and maintain key marketing strategies and relationships with community partners and stakeholders
  • Liaise with organizational leaders and community partners to design prevention strategies to improve health outcomes for older adults at risk of falling.
Risk, Health and Safety Management

  • Identifying and reporting health and safety incidents and concerns in a timely manner to the appropriate supervisors and/or funders, documenting incidents in EasyCare and escalating appropriately to the designated supervisors as outlined in the Client Safety Reporting policy (C
  • Participating in health and safety processes and procedures
  • Participating in maintaining a safe workplace environment by cultivating a positive safety culture and encouraging best practices to promote both staff and client safety and wellbeing
  • Participating in all health and safety training initiatives on a regular basis.
  • Taking proactive action against client incidents within your scope of practice.
  • Developing a plan to identify, manage and/or minimize client safety risks or situations in adherence with risk management operations policies
  • Assessing the severity of an adverse client safety/risk event and determining the best followup and developing an action plan following the event. Collaborating with funder (ex. HCCSS) and following any additional processes as required.
  • Calling emergency services (911) when the client is at an immediate risk of harming themselves or others, or if there is a serious injury and/or imminent harm.
  • Evaluating any potential hazards and identifying clients at risk for adverse health and safety events, taking preventative measures when necessary to minimize reoccurrence.
  • Reporting all safety events impacting clients, caregivers and families in a timely and honest disclosure.

Qualifications:


  • Completion of Degree in Kinesiology, Occupational Therapy or Physiotherapy.
  • Current registration with a provincial professional association (College of Kinesiologists Ontario, College of Occupational Therapist of Ontario, College of Physiotherapists of Ontario) or equivalent.
  • Minimum 1 year of working experience, with demonstrated experience in developing and implementing programs with older adults
  • Demonstrated experience in implementing exercise programs with other adults and physically able to lead exercise programs including the completion of assessments.
  • Up to date knowledge of best practices for falls prevention for older adults
  • Work independently and collaboratively within the organization as well as with community partners
  • Demonstrate solid interpersonal, communication, problemsolving, time management and organizational skills.
  • Proficiency in the use of computers and proficient in Microsoft Office and other similar software products.
  • Valid Driver's License and access to a personal vehicle for business purposes required.
