Your Opportunity:


Reporting to the Director, Primary Care Integration, the Project Manager will be responsible for supporting high-value strategic initiatives; supporting the development, implementation, and management of projects with cross-functional teams to achieve the organizations strategic goals.

The primary focus of the role will be to support healthcare integration initiatives that contribute to the continuum of connected healthcare services and improve patient experience through efforts identified across health and social service organizations.


As a Project Manager working in this Family Health Team, you will:

  • Contribute to the organization's efforts and initiatives to enhance HFHT mission, vision, and values.
  • Make recommendations on services, policies and programs while striving to continuously improve processes.
  • Ensure consistency through the implementation of consistent tools and methodologies while maintaining appropriate documentation which clearly illustrates project progress and success at completion.
  • Prepare, review, and submit reports to department staff, SLT, the Board, and external agencies that are clear, concise, transparent, and accurate.
  • Ensure that patient experience and improving patient care remains central, building equitable, efficient, and quality healthcare services.
  • Support the broader HFHT healthcare integration initiatives, while also balancing consistency and standardization across, and unique needs of, the designated geographical neighbourhoods.
  • Provide project management support/expertise across the organization, as needed.
  • Facilitate and lead in supporting strategic initiatives.
  • Develop project plans for projects, evaluation, implementation, and communication.
  • Proactively take appropriate action to keep the project on track by identifying risks/barriers to success, scope creep, schedule challenges, and offering mitigation strategies.
  • Chair/participate in relevant working groups for organizational projects and initiatives.
Lean Six Sigma methodologies and tools
Problem identification and root cause analysis
Brainstorming and focus group facilitation
Change management tools
Statistical analysis including run and control charts
Performance Measurement

  • Provide consultation/guidance regarding appropriate indicators, targets, and data sources to measure performance of projects.
  • Prepare and present reports that analyze trends.
  • Interpret data related to projects to inform decisions of stakeholders, including Senior Leadership.

You bring:


  • Undergraduate degree in a relevant health related field required, master's degree preferred.
  • Regulated Health Professional and a member in good standing with the relevant professional college or association (Social Work, Psychology, Nursing, OT, PT, etc.) is considered an asset.
  • 35 years of practical related experience in a primary care or community setting is considered an asset.
  • Project Management Professional (PMP) or Lean Six Sigma black belt designation or equivalent experience.
  • Excellent written and oral communication skills including a highlevel of comfort with medical jargon and language.
  • Excellent time management and prioritization skills.
  • Competency in project management, process improvement, business planning, performance measurement and evaluation.
  • Demonstrated ability to attend work on a regular basis is required.
