The Opportunity
This management position oversees interdisciplinary teams supporting RCTP (Regional Community Transition Program) post-acute program at Carewest Dr. Vernon Fanning Centre.

The role includes overseeing day to day operations, and innovative program planning, implementation, and evaluation meeting the needs of our clients and families, while ensuring effective and efficient resource utilization through strong collaboration with internal and external stakeholders.

The role includes coaching and mentoring an inter-disciplinary team to achieve safe, quality care, while meeting the standards of practice for post
- acute inpatient rehabilitation care. The role is informed by regulations and guidelines, while adhering to Carewest policies, procedures, protocols, and best practices. Central to this role is the embodiment of the Carewest values of caring, building healthy relationships, practicing excellence and encouraging others to do same, taking accountability for actions, and partnering in effective teams. Promotes mutual respect and trust when assisting team members to analyze and solve clinical problems.


Program Summary

Rehabilitation and Community Transition Program (RCTP): If you are a highly motivated nurse leader, with advanced skills and experience, our Carewest, Dr Vernon Fanning Centre, RCTP Inter-disciplinary Team would like to hear from you. Our short-stay program supports medically stable clients, who do not need acute care level of service, but may require additional assessment, recuperation, and therapy before returning home or moving into another care setting. Clients receive treatment to help improve their level of functioning to the point where they can manage at home, or in another appropriate environment. Clients who have hemodialysis needs are primary treated at the Dr Vernon Fanning RCTP site.


Qualifications & Experience:


  • Bachelor's degree in a healthrelated discipline required; master's degree preferred
  • Current registration with appropriate professional licensing college
  • Minimum 3 years clinical operational leadership in a health care role.
  • Demonstrated ability to manage change and create innovative solutions for complex and diverse issues.
  • Understanding of AHS Model of Care for Rehabilitation ifrehabmodelof ) or other Rehab Model.
  • Leadership skills, including demonstrated ability and comfort with decision making responsibilities, coaching, and teaching, and the ability to inspire and build confidence in others.
  • Ability to lead staff to meet the standards of practice outlined by professional licensing bodies (CRNA, CLPNA, CAOT, CPTA, ACSW).
  • Ability to foster a climate of cooperation, and build relationships with other postacute units, transition services, and other committees and partners internal and external to Carewest.
  • Knowledge of system processes for care transitions.
  • Client and Family focused with a commitment to providing high quality services.
  • Ability to continually analyze effectiveness of strategies and implement quality and process improvements.
  • Able to demonstrate competence in the development of new programs and services to meet the needs of the population.
