About the Opportunity:

Nova Scotia Health has established a new Care Coordination Centre that will improve how we coordinate the movement of patients throughout their healthcare journey.

It will use artificial intelligence, predictive analytics, and patient status to provide a real-time view of access to care, patient flow, and health system resources to support decision-making.

It will also generate actionable insights to drive continuous improvements in how healthcare is delivered to improve the quality of patient care.

Care Coordination Centre hubs across the Northern Zone supports teams to identify issues and find solutions together, using real-time data.

This will translate into improved patient access and flow, efficient bed management and discharge planning, better scheduling of staff, and more collaboration across health zones across the province.

Reporting to the Director of Patient Access and Flow in Northern Zone, the Patient Flow Manager is a fully realigned and integrated role in C3 within the Northern Zone C3 hubs that incorporates the 24/7 patient flow and after hours administrative functions.

Using sound principles of Utilization Management the role is responsible for facilitating patient placement to appropriate services by identifying and overcoming delays in admission, transfer and discharge.

This position involves liaising with the Emergency Department, Booking Officers, Health Service Directors, Managers, Staff Physicians, Discharge Planning Coordinators and other Health Care Agencies within and external to NSHA and possibly in other provinces.

While the day focus of this role, when full administrative supports are on site, will be on clinical flow, after hours and on holidays and weekends the role will provide a resource for health professionals and support departments and the liaison for Senior Management and Executive.
