Clinical Social Worker - Hospice Palliative Care Specialist

As a Clinical Social Worker for the Tertiary Hospice Palliative Care Unit, you will provide therapeutic counseling services to patients and families experiencing complex circumstances related to illness or end-of-life issues.

The ideal candidate will have experience working with hospice palliative care patients and their families in an acute care environment. You must be able to conduct comprehensive social work and psycho-social assessments, develop and coordinate educational programs, and collaborate with interdisciplinary teams.

• Provide comprehensive therapeutic counseling services for patients and families admitted to the unit
• Conduct comprehensive social work and psychosocial assessments of THPCU patients and families
• Develop, coordinate, and deliver in-services and presentations in collaboration with the interdisciplinary team
