Reconnect Community Health Services is one of the largest not-for-profit health providers in the west end of Toronto. We provide services for seniors, caregivers and those experiencing mental health and addictions concerns. We support over 7000 clients each year and are proud to be an integral part of our community.


Successful applicants to this role will be knowledgeable in project management tenants and norms, have experience working within the healthcare sector, will be organized and detail oriented, will have excellent time management skills and will be able to build relationships with a variety of stakeholders.

Good communication is key, as is the ability to work in a fast-paced, changing environment.


Responsibilities include, but are not limited to, the following:

  • Provides support to the leadership team and participates in the planning, implementation and evaluation responsibilities related to the Academy and other projects;
  • Develops and maintains relationships with partners and diverse stakeholders in relevant projects;
  • Review and develop contracts with trainers;
  • Manage trainer and other stakeholder relationships;
  • Ensure consistent outreach and participation in the Academy;
  • Delivers all services in a professional manner and according to established guidelines and associated benchmarks;
  • Supports the leadership team in conducting stakeholder meetings and forums in order to solicit feedback, input, and expectations, and incorporates recommendations into project plans;
  • Plans all project timelines, milestones and deliverables using appropriate methodologies and techniques;
  • Develops mechanisms for tracking project progress and reporting to stakeholders;
  • Provides regular updates to the leadership and other stakeholders as required through project reports, presentations and other mediums;
  • Responds to issues identified by stakeholders;
  • Anticipates potential barriers to success and mitigates risk including the creation of contingency plans;
  • Works collaboratively with other internal and external project teams;
  • Other duties as required.

Technical and Behavioural Competencies Required:


  • 23 years of experience in a project management role preferably within a health services environment;
  • Post-Secondary Education in health care, social services, an information technology related field or equivalent experience;
  • Selfdirected and independent, but also experienced in working effectively as a member of team, which includes internal and external stakeholders;
  • Above average written ability; ability to analyze written reports, complex documents and publications;
  • Strong experience in developing and managing multidisciplinary and multiorganizational projects;
  • Demonstrated ability to coordinate multiple tasks and keep stakeholders effectively involved in the process;
  • Excellent time management and work prioritization skills with an ability to effectively manage multiple high impact priorities;
  • Able to interpret data and use it to develop effective and appropriate health care programs;
  • Excellent verbal, written and presentation skills;
  • Strong interpersonal and relationship building experience, including a proven ability to build and develop effective relationships with health service providers and community partners;
  • Strong analytical, problem solving and critical thinking skills;
  • Advanced level proficiency in Microsoft Word, Excel and PowerPoint;
  • Knowledge of and sensitivity to gender, race, sexual orientation, age and multicultural issues.

Special Requirements:


  • Criminal background check;
  • Works a 37.5 hour workweek, Monday through Friday and as needed after hours and on weekends.
  • Work hours may vary depending on the needs of the project and the agency.
  • May provide services at the Reconnect offices and at satellite locations.
  • Attendance at outside meetings and special events required from timetotime.
