Summary:


The Clinical Coordinator, Primary Care, Foundry Granville provides first line supervision, coordinates the day to day operations of the Clinic and ensures that operations comply with the applicable Health Canada requirements.

In collaboration with all health care team members, coordinates client flow activities by reviewing clinic processes and developing protocols for improving client flow.

Provides clinical direction and coordination of clients and assists the Manager with a variety of administrative activities to ensure efficient and optimal use of client care resources.

Liaises with designated team members/staff and supports interdisciplinary practice, quality improvement initiatives and supports interdisciplinary integration of services.


Qualifications / Skills and Education:

Education
Registration with the British Columbia College of Nurses and Midwives (BCCNM) as a practicing RN or RPN registrant.


Three years' recent, related nursing experience in the Community Health Nursing including one year's related leadership/supervisory experience or an equivalent combination of training and experience; completion of the Point of Care HIV Testing course; and successful completion of the BCCDC Immunization certificate.

CRNBC Sexually Transmitted Infection (STI) Management certification, or evidence of enrollment

Broad knowledge of the local Community and regional initiatives that may impact patient care, resource access or practice guidelines in this local clinic setting.

Duties and Responsibilities:


Ensures staffing levels match client care needs by performing duties such as scheduling and coordinating the use of staff, equipment and other resources on an ongoing basis, assisting staff in workload decision-making, authorizing overtime and providing clinical direction and ongoing monitoring, evaluation and follow-up of work.


Evaluates the provision of direct care to clients and their families in accordance with established standards, policies and protocols and the vision and values of Community Health Services through methods such as assisting staff to operationalize standards, reviewing/auditing relevant documentation, and interviewing clients/families to investigate clinical components of incident reports, provides intervention as required, and ensures care standards are followed.

Refers performance related issues to the Manager.


Provides direct health care services to designated clients by performing physical assessments, collecting specimens as necessary, referring clients to the physician for further examination if necessary and administering prescribed treatments and medications.

Ensures clinical prioritization of clients requiring service by allocating staff resources to perform prioritization duties.

Communicates with a wide range of health care professionals within and outside the Community health Centre to provide information on the client's status, arranging access to a variety of specialized services.


Works with the Medical Coordinator and clinic physicians in determining appropriate client/physician assignments and help facilitate the smooth transition from one care provider to another as necessary.

Provides input to the Medical Coordinator in ensuring appropriate and safe physician staffing levels.

Monitors and coordinates external and internal client flow through the system with physicians, nurses, front desk staff and other care providers to ensure timely and effect
