Mental Health & Addiction Supportive Housing Program (MH&ASH) is a rent subsidy and case management
'Housing First' program designed to provide intensive case management and housing support to persons who:

  • have severe and active substance use & mental health challenges.
- are experiencing homelessness or precarious housing.
- high intensity service use of emergency departments, hospitals, withdrawal management and/or the

criminal justice system.
The program works closely with Fred Victor's Addiction Supportive Housing (ASH) case managers who work to
support program clients in the community. The programs manage 32 units of rent-supplemented housing, and
several individuals receive only case management support. The case managers provide intensive case management
to support 10 to 15 clients.

determinants of health and the principles of anti-racism, anti-oppression, equity, excellence. We are an agile team that
promotes learning and innovation. We provide high quality case management services and housing stabilization
services to individuals experiencing homelessness in the community and dealing with severe & persistent mental health
difficulties.

What You Will Do:


  • Provide both onsite and communitybased assessment and longterm case management services to
individuals living in units in various locations. Services are short-term and intended to connect individuals to
needed services to support their ongoing wellbeing.

  • Provide intensive case management to support clients to stabilize their living situation. Identify client's goals
around substance use, mental health, and treatment, which are all part of a comprehensive treatment
planning process which utilizes a harm reduction approach.

  • Supports clients in the acquisition of skills in daily living, developing coping strategies and promoting new
behaviours that improve wellbeing and safety.

  • Ongoing screening, assessment, monitoring, referral, advocacy, counselling, education, and life skills training
are all part of this program.
Case Managers work as part of a team to meet clients' housing and other social and health needs. The program
works closely with a variety of health care professionals & coordinates services in various settings including, but not
limited to, hospitals, housing, incarceration systems, various treatment settings, etc.


What You Bring to the Team:

  • Post-secondary education with a degree or diploma in Social Work, Social Services, or Health.
  • A minimum of two years of experience within the homelessness services sector, preferably in concurrent
disorder services with an emphasis on providing support through a recovery and strength-based approach.

  • Working knowledge of the Housing First, Harm Reduction, and Trauma-Informed frameworks.
  • Engaging effectively with individuals experiencing serious mental health issues and/or addictions, cognitive
difficulties, brain injury or primary health care issues.
