- Develop and implement individualized service plans for clients.
- Provide crisis intervention and support services.
- Provide advocacy and support to clients.
- Maintain accurate and up-to-date client records.
- Collaborate with other professionals to coordinate services for clients.
- Provide referrals to community resources.
- Participate in team meetings and other professional development activities.
- Minimum of one years of experience in case management.
