Position Summary

As a member of Peter Munk Cardiac Centre team, the Registered Nurse, Clinical Coordinator participates in managing the Heart Function remote patient monitoring program Medly for General Internal (GIM) heart failure patients at TWH.


Duties

  • Attending the GIM heart function clinics inpatient floors and Heart Function clinics and enrolling heart failure patients in the Medly program
  • Providing basic Medly tech support and escalating to Global eHealth and Innovation the Centre for Digital Therapeutics (CDTx) at UHN as appropriate
  • Managing the daily (Monday-Friday) Medly alerts in partnership with the Heart Function Nurse Practitioners and Physicians.
  • Communicating with patients and families and reviewing proper use of Medly; prioritizing alerts and escalating to the NP/MDs as appropriate
  • Providing patients and families with heart failure education on topics such as signs and symptoms of heart failure and selfcare management of heart failure
  • Uses Medly to support Guideline-Directed Medical Therapy for heart failure and titration of medications
  • Provides education/training/mentorship to external facilities as appropriate
  • Documents appropriately, conversations that occur with patients/families
  • Offboards patients from Medly once stable and attain GDMT, ensuring safe discharge and providing clear information for return to the program, if needed
  • Participates in refinement of the Medly program as appropriate
  • Applies best practice guidelines to ensure that patients receive care according to provincial standards

Qualifications:


  • Current registration with the College of Nurses of Ontario
  • Bachelor of Science in Nursing required
  • Current BCLS Certification
  • Minimum five (5) years Cardiac +/or Cardiac Surgery experience required
  • Critical Care or Coronary Care Certification preferred
  • Ambulatory experience preferred
  • Remote monitoring experience a definite n asset
  • Knowledge and experience with heart failure guidelines and remote titration of HF patients an asset.
  • Experience working with community health providers preferred
  • Experience in change management
  • Strong organizational and time management skills
  • Excellent decisionmaking and problem
