Since 1985, Universal Rehabilitation Service Agency, URSA, has been a Calgary based non-profit agency that works to meet the needs of individuals with disabilities in community settings.

We continue to break down a multitude of barriers for these individuals and help them thrive in the community.

URSA has a variety of programs ranging from 10 hours a week in-home support, to 24-hour staffed residences, and a variety of day programs.


Reporting to the Manager, the Community Care Coordinator is responsible for duties directed towards the total development and care of persons with developmental disabilities, brain injury and complex medical needs.

Duties include the assessment of individual needs and the design and implementation of care plans.

This team member assumes responsibility for coordination of the care and service of the individuals within the Agency and ensures training is complete to meet each individual's unique medical and complex care needs.


As the Community Care Coordinator, a typical day might include:

  • Coordination and facilitation of a transdisciplinary team and identification of additional supports that may be needed for the individuals in service.
  • Ensuring mandatory and ongoing training and development is provided to all employees related to medication administration and individual care/procedures.
  • Staying up to date on all relevant health concerns that require additional support and identify when additional consultations need to occur with external health care providers.
  • Ensuring all recommendations for changes/improvements to dietary issues/therapeutic feeding techniques, etc. are forwarded to the appropriate specialists.
  • Monitoring program operations to ensure compliance to all policies, procedures, legislation, accreditation and Health and Safety standards.
  • Ensuring all sites complete a medication audit twice per year. At minimum, the Community Care Coordinator will audit each site annually and will monitor, review and oversee all medication audits completed by the Medication Committee.
  • With the support of the Manager, facilitating and chairing regular medication committee meetings throughout the year.
  • In conjunction with HR, ensuring that staff maintain current status in all certifications related to individual care and medication administration.
  • Developing and monitoring effective documentation systems that meet specific standards of service related to the medical needs and care of the individuals in service.
  • Recommending, reviewing and revising changes and additions to policies and procedures related to individual safety, medication administration, and nursing procedures as needed.
  • Completing trainings on required care or medical skills within the LPN or RN Scope of Practice.
  • To be knowledgeable of available resources and facilitate training and orientation to the changing medical needs of the individuals in service.
  • Ensuring coordination with interagency programs and other Agency related services such as the Community Paramedic Program and Practicum students from the healthcare field.
  • May liaison between URSA and Alberta Health Services related to homecare involvement and Outbreak Management as required.
  • Ensuring relevant and timely exchange of information between all team members and supervisor.
  • Participating in oncall rotation and providing meaningful support to staff who access oncall after hours, ensuring that all necessary protocols and documentation occurs.
  • May perform other duties as required.

General Requirements:


  • A degree or diploma in Nursing and are in good standing with CLPNA or CARNA.
  • Experience working with individuals who have high medical needs.
  • A minimum of 3 years supervisory experience.
  • Exceptional communication skills and a proven track record of coaching and mentoring a team.
  • Must have a valid class 5 drivers license, provide proof of $2,000,000 dollar liability insurance, business rider insurance and a clean drivers abstract with 3 years claim free driving in Canada
  • Must be able to work 1 evening shift a week
  • Must be able to work 1 weekend day shift a month

$70,000.00 per year
