Position Summary
The Practice Facilitator is a natural relationship builder between member physicians, their teams and Mosaic Primary Care Network.

With a focus on bi-directional communication, they are adept at building trust and negotiating with stakeholders to identify optimum strategies to advance the continuous improvement of clinical processes.

They then act as and/or identify and engage appropriate resources to support implementation of these processes in order to influence positive patient outcomes and access appropriate health systems supports such as MPCN programming.


The Practice Facilitator provides feedback to MPCN operations regarding physician member experiences with services and programming, barriers to implementation success, and suggestions for improvement.

They participate in Plan, Do, Study, Act (PDSA) to ensure that the Patient Medical Home remains as the foundation of decision making.


The Practice Facilitator will consistently use their skills in Health Change Methodology, Model for Improvement, and change management (preferably Prosci) in their relationship management internally and externally.

They will be able to translate complex technical information from a variety of sources and illustrate how it supports member physicians and their teams to reach clinical practice goals.


The Practice Facilitator role requires frequent travel between MPCN central clinics and member physician clinics to support a tailored implementation approach to participating clinics.


Key Responsibilities

  • Relationship Building:_
  • Build and maintain trust and collaborative team relationships with physician members and their teams to effectively contribute to Mosaic PCN's goals and the Patient Medical Home (PMH) objectives
  • Promote Mosaic PCN programs and services to physician members so that they can appropriately serve Mosaic PCN patient population
  • Interact with physician membership to gather informal/anecdotal feedback for improvement of current services and development of new programs
  • Collaborate with Mosaic PCN clinical operations to address physician concerns and opportunities for improvement to maximize patient outcomes
  • Customer Focus:_
  • Assess current status of clinic to ensure implementation of the PMH program aligns with the clinic's identified needs and context
  • Identify and articulate PMH value to physician and clinic teams
  • Foster high quality service with strong commitment to quality and process improvement and patientcentered care
  • Formally meet with each clinic at least monthly to discuss successes, barriers and needs then supports operationalization of QI and office efficiency needs to support forward momentum
  • Quality Improvement, Training & Evaluation:_
  • Mentor and train frontline staff in participating member physician clinics on the Patient's Medical Home and requirements of related programs.
  • Participate in the design of and facilitation of group training events and clinic onboarding
  • Engage EMR Specialists as required to provide expertise and training on the functionality of the various Electronic Medical Records and Netcare
  • Support the creation of standardized reports at the clinic and program level and delivers their content to the physician/clinic in a way that adds value to their practice
  • Support clinics to introduce measurement and extract data from their Electronic Medical Record and assist with basic interpretation and analysis of the data
  • Advance understanding and interpretation of HQCA reports and other health system utilization documents and the ability to translate data into valuable practice level QI projects.
  • Facilitate and support physicians and teams in developing quality improvement projects.
  • Facilitate quality improvement by engaging physicians and multidisciplinary teams (MDT) within the member clinics.
  • Participates in the development and measurement of program outcomes.
