OVERVIEW
The Nurse Practitioner (NP) - Respirology/Progressive Weaning Center is a nurse who is prepared at the graduate level and is registered with the College of Nurses of Ontario in the Extended Class. They have experience and knowledge about Patients with prolonged Critical Care stay. The NP possesses the competencies to autonomously diagnose, order and interpret diagnostic tests, prescribe pharmaceuticals and perform specific procedures within their legislated scope of practice. The NP Respirology/PWC uses in-depth nursing knowledge, clinical expertise, research and educational skills to support the care of all patients with complex respiratory illness and prolonged mechanical ventilation at Michael Garron Hospital (MGH). The NP Respirology/PWC will work in collaboration with physicians and other medical staff and members of the interdisciplinary team as detailed in the NP's Collaborative Practice Agreement.
The NP Respirology / PWC will work collaboratively with both internal and external partners to facilitate admission, smooth transition, discharge and ambulatory follow-up of complex respiratory patients, including those with persistent and chronic critical illness.
The NP will:
- Assess and treat patients admitted under the Respirology / PWC program within the Medicine Health Service at Michael Garron Hospital, providing care for patients within their legislated scope of practice.
- Work in partnership with stakeholders, physicians, nurses and other inter-professional members to develop a culture of collaborative patient centered care and to ensure safe care transitions for clients and families.
- Demonstrate clinical excellence through all four domains of NP practice: clinical care, leadership, education, and research.
- Provide coaching and mentoring to staff that enhances the provision of quality care using evidence-based practices and research.
EDUCATION
Master in Nursing / Nurse Practitioner (MN-NP) or Master of Science in Nursing with successful completion of a post Master Acute Care Nurse Practitioner Certificate Program from a recognized school of graduate studies required- Adult Stream.
- Current College of Nurses of Ontario registration in the Extended Class (EC) as an NP (adult) or NP (primary health care) required.
Critical Care experience is required.
- CACCN (Canadian Association of Critical Care Nursing) certificate is an asset.
-BLS required.
QUALIFICATIONS
1-3 years of demonstrated experience as a Nurse Practitioner required.
- 3-5 years of critical care experience.
- Demonstrated sound knowledge and skill with adult education approaches and methods.
- Demonstrated in-depth expertise regarding clinical assessment and acute care needs of patients with acute and chronic respiratory illness.
- Demonstrated ability to work in a self-directed manner with well-developed leadership, critical thinking, organizational and problem-solving skills.
-Demonstrated interpersonal, team development, and conflict management skills.
- Advanced-level knowledge of patient safety, change leadership, project management and evaluation an asset.
- Demonstrated strong communication and interpersonal skills, with the ability to work collaboratively with physicians, inter-professional team members, and other stakeholders.
- Possesses highly developed analytical, problem solving and critical thinking skills.
- Excellent oral, written, and presentation skills, with demonstrated competency in information and communication technology.
- Demonstrated excellent organizational and coordination skills and the ability to prioritize multiple competing workload demands.
- Demonstrates an ability to provide leadership, support and critical analysis in stressful, complex clinical and emotional situations.
- Experience in Case Management preferred and knowledge of community resources.
- Embraces a philosophy of patient and family centered care.
- Demonstrates leadership in clinical excellence.
